help with time change please

Tommorw i will be home 1.5'hrs late for Buddy's pm shot.He's currently getting 1.5 u
How much more should i give him to make up for the late shot? Was thinking a fat 1.5---is that too much? Thanks
 
If you have to give a late shot, you don't increase the dose to make up for the lateness. Leave the dose the same.

Is there a possibility you can give a split dose in the morning? You would give half the dose at the usual shot time, then the other half at the time which is 12 hours before you will be able to give the evening shot. That extends the duration and the PM shot will not have to refill much, if any, shed. You also give only the regular dose at the PM shot, unless you are going to have to give an earlier shot the next morning to get back on track.

I can check in again in the morning, I leave around 8AM central time.
 
If I have to shoot up to 4 hrs late and can't slowly move the shot times (like when I had a job and stayed downtown for a play after work), I'd shoot the regular dose (full dose) when I got home and a reduced dose the next am by more or less, depending on when I was shooting it - how many hours after the last shot.

The big question is if you will be able to shoot late the next morning or not? If not, then you need to reduce at least the am shot. Or you could reduce both to 1.25u just to be on the safe side. Expect some wonkiness for a few cycles.

But please explain, just so I know...I thought an early shot was like a dose increase, and a late one like a dose decrease, so i assumed i would need to shoot abit more if I am 1.5 hours late with shot? why the same dose tonight? at what point do i increase/decrease if I am not on time? Thanks again.

You are correct that an earlier shot is like a dose increase, so you would reduce if you were giving an earlier shot. It has to do with the shed. An earlier shot could overflow the shed.

When you need to do a later shot however the problem really isn't less insulin, it is lack of duration, but you can't give more insulin to make it last longer because of the way insulin works. Giving a larger dose to try to make it last longer may put the cat in danger of hypo because it will send BGs lower. So you need to do something to extend duration without risking hypoglycemic BGs. The answer to that is splitting the usual dose in half and giving one half at the regular shot time and the other how ever much later you need to extend duration. For example, if you normally give shot at 7AM/7PM schedule, but you can't give the PM shot until 9PM you would give half the dose at 7AM and the other half at 9AM, then the full dose at 9PM.

Unless you need to get back to 7AM/7PM schedule you do not need to change the dose if you are doing splits, other than to split it in half. If you need to get the shot earlier, you would reduce it the cycle BEFORE you give the earlier shot, AND for the earlier shot. Say you want to get back to 7AM by Thursday. You could stay with 9AM/9PM tomorrow, but give the reduced shot tomorrow 9PM. Then the shot at 7AM on Thursday is also at the same reduced amount.

The first time someone moves a shot time earlier by this method it would be a very good idea to test over that day to see the result. I did it enough I know it worked for Gandalf without causing a problem, but that darn ECID says we should use caution and get the data so you know it is safe to do it again that way.
