Help with Starting Dose (switching insulin)

We are about to switch from Lantus (vial) to Levemir (pen).

What should be the starting dose if we are at 4 units Lantus?

Is the protocol the same (TR or SLGS)?

Can the pen be left at room temperature for less than 24 hours? Will that affect the insulin? It was shipped and arrived warm, but I immediately put it in the fridge.

Both lantus and lev are depot insulin’s and dosed according to nadir. I don’t see any mid cycles tests recently so can’t advise. Cats often drop lower at night and you don’t have any tests since mid April at night. I suggest getting a +3 or before bed test every night and mid cycle test day and night when possible. Maybe at least do a curve on your days off?
