Hello I need help ... my cat has been on lantus for 3 years and it was not bad at all, it was never regulated ... a couple of weeks ago the cat stopped climbing where it always did but today the cat does not even want to walk screaming in pain. The vet says it could be diabetic neuropathy because of the way he walks. But he doesn't want to walk or be touched. She suggested us to change to prozinc to see if the cat will get better but if my cat was taking 1.5 of lantus... now she wants me to give him 1.25 of prozinc with the same syringe the cat woke up with 376 and she told me to only prick him once a day... I am frustrated I keep crying the cat does not even want me to touch him he is in a lot of pain... can anyone help me?

Could Sinver have injured himself? Did the vet do X-ray?
Also, you know it’s not safe to only check his glucose once a day!
Bad advice from the vet, but safe at all!
I’m hoping you can get some suggestions here to help!
When did you see the vet?

If he’s in that much pain you should perhaps take him to the emergency vet 😢

I doubt a change of insulin will help with your cat's pain. Something is clearly wrong. Insist that the vet do x rays and bloodwork. Ask about arthritis. Ask for pain medication.

If you decide to change insulin, you can use U100 syringes with Prozinc **but** you need to convert the dose. To give 1.25 units of Prozinc you need to measure to the 3 unit line. Here's the conversion chart for reference Insulin Conversions - Felinediabetes.com If it's too confusing then you must buy U40 syringes.

Xelo, I’m very sorry that Sinver is not feeling well. From your last lab work, the potassium is quite low, and I am not sure whether the vet started Sinver on potassium supplements then, but at that low level he will very likely be showing symptoms of hind leg weakness. You want a value definitely above 4 and he is at 2.5.

Second and more concerning, is the pain. I am very worried about possible saddle thrombus, are either or both of his hind foot pads cold or blue? I agree with Staci, please consider taking him to a vet, this sounds very serious.

I am not an expert, but i just wanted to share with you my personal experience - my cat did not have pain when he had diabetic neuropathy - even at its worst and he could barely walk straight.

I have been taking the cat to the vet for two weeks. They did ultrasounds on his bladder, they saw nothing. I still have a blood test pending, but the last one showed a potassium level that went up to 3.1 and the value was 4. They also did x-rays and they only saw a little bit of osteoarthritis. He has lost all muscle mass. Last night he didn’t want to eat. We gave him pregabalin, which is a sedative because he was crying a lot at night from the pain. The vet couldn’t find a reason. He told me it could be neuropathies, that’s why I changed delantus to prozinc because he says it’s very unbalanced. I don’t know if changing his insulin would be a good option, given how he is. I need some advice. He barely eats or drinks water. The blood tests didn’t show any changes in his body. Any advice on any tests I could ask the doctor for?

painkillers don't work for him,

My cat squeals in pain when he tries to move. He spends the whole day lying down without moving so that he doesn't hurt... I don't think it's diabetic neuropathy, there's something else. He's started to stop eating. I'm sad because it's frustrating not being able to help him and seeing how he rages in pain.

Has the vet suggested an ultrasound of the GI system? Preferably by a vet certified in this. When my cat had ileus her potassium went low and she could barely walk. She also didn't want to eat. But was not in pain. I'd be more concerned about saddle thrombus like Christie suggested. Maybe try a different vet. I had one cat with saddle thrombus that one vet completely missed. I took him back to the clinic to a different vet the next day, who diagnosed it right away.

No his paws are not blue and they are not cold... what test do you advise me to ask the vet for?

What tests should I ask the veterinarian for?

Just so you know, pregabalin isn't a sedative. It's a form of gabapentin and is used to treat nerve pain. At the very least, ask to try buprenorphine which is an opiate and see if the pain diminishes.

It all started about a month ago when I noticed that Sinver's spinal vertebrae were noticeable but it was nothing significant. We went on vacation and he didn't have any symptoms, he was a happy cat... as soon as we put him in the transport to fly back home he seemed like a different cat, he came out crawling with his head down and he started to stop going up to the places but he walked, a week later he barely wanted to get up but without pain, but this week he started to shiver when he moved, they did several tests, x-rays which according to the vet said it could be a little arthritis but with the drugs he didn't get better. Today I notice that he has lost more muscle mass all over his body, I imagine it's because he has been lying down for two weeks without moving, he barely wanted to drink water and when I took him to the litter box to do his needs he started to scold me, I didn't want me to go near him. He wanted to stay hidden. Yesterday I went to another vet to discuss the case, and he gave me cortisone. It seems like he's not in that much pain because when he moves, he doesn't start to urinate. I need advice on what other tests I could ask my vet for to help me get a diagnosis.

The vet gave me a medication called tramadol, which is from the opiate family. Another vet gave me gabapentin in case it was diabetic neuropathy. But yesterday, with the cortisone and without the gabapentin, the pain wasn't as bad. Do you think I should give him this medication for the pain?

Could someone tell me if the factor of having a low potassium level in the blood tests could be the cause of this disease? Hypokalemia, apparently has the same symptoms that my cat describes. I'm going crazy looking for information.

If the cortisone helped, at least a part of the problem is inflammation. Cortisone is a steroid. What that means is that it will likely cause a rise in Sinver's blood glucose numbers. I wouldn't worry about the effect on blood glucose since getting the pain under control sounds like it's more important.

The cortisone helped him a lot, he is not raging with pain, it seems to have reduced his inflammation, the vet suggested a third analysis tomorrow, do you think I should do an MRI? There we will be able to see if he has something in his spine that affects his paws? He is quite calm if he does not move ... when he moves is when he complains ... I have not had time to see if he uses the sandbox ....

I do not have enough money the MRI is worth between tests and everything about 3000 euros at the moment I can not afford it.
