Okay first things first, the dose has been raised by way too much each time it was raised, and secondly there is way too much of a gap between when Blinky was tested from one time to the next, in days not the number of hours between tests on a particular day.

What is his current diet? Food can play a huge role in managing this disease as well as the right insulin dose. I also use Levemir for my one girl still insulin dependent as well as for the one that is now off insulin. It is the demeanor change that you noted on your spreadsheet that has me the most concerned, because that isn't typical with a Levemir cat or for that matter any diabetic cat when they are well regulated.
The problem is too much insulin and not enough can look exactly the same if you aren't home testing daily to catch the lows that too much insulin can cause, which then triggers the liver to go into self protect mode and jumps them right back up into high numbers again and it becomes a vicious cycle.
My Cassanova was on 11u of Levemir and only having curves run at the vet's by his previous owners, when I adopted him I could have described him very much like you did your Blinky. Very poor clinical signs, lousy coat condition, lethargic etc. Switched his diet to a low carb canned wet food and started testing him at home, 8 months later he is insulin free and looking smashing.
