Help Dose all over the place - confused

A bit confused tonight. Finally moved Ernie to 1 unit to try to get him to a point where I can shoot the same am/pm unit amount but tonight after giving his 1 unit in the am, he’s actually way lower than he has been for ant PMPS. He’s at 6.4 tonight which is really the lowest he’s been recorded ever.

Checked his pee for ketones which was negative. Not sure what’s going on I hoped giving the lower am dose would mean his sugars would be high enough tonight for his insulin but it looks like I can’t even give a shot at all! So confused right now.

Just waiting an extra 30 min to test again…

Normally if you could test at night, I would say to give a dose and see how it goes. But as you can’t test at night it makes it hard.

He probably hasn’t eaten that well today he had half a can of fancy feast pate and about 1/4 cup of kibble if that. Usually he would have 1/2 cup of kibble per day. But I’ve been trying to work in the fancy feast Pate lately but he only really eats half a can watered down.
 
As you are doing SLGS and can’t test tonight I would skip the dose. And it’s good there were no ketones.
Try 1 unit again tomorrow if he’s high enough and if the same thing happens tomorrow night we might have to reduce the dose to 0.75 units and see if that works for both cycles.

Make sure he eats enough food every day. I know it can be hard doing the swap to wet food but it’s very important a diabetic cats eats well every day.
Have you tried other low carb foods such as Weruva? Or feline narurals?
 
No problem. I meant to look a bit earlier because I know you shoot at 10 but I got caught up feeding my lot and one is not eating well either.
Oh no it’s terrible when that happens. I’m trying not to resort to the Mirtazapine too often but it’s like he needs a kick start with it every few days. I’ll see how he goes tomorrow if he doesn’t eat more kibble overnight. Might need to give him a dose.

Do you have an antinausea medication such as cerenia or ondansetron (zofran). If he is nauseated it is better to give an antinausea then a bit later the appetite stimulant. If he needs the Mirtazapine let him have it.

He’s on odansetron half a tablet per day and Gabapentin and Buprenorphine atm. This week I’m phasing out the Buprenorphine so that it’s once a day. The Gabapentin till keep it twice a day as that’s what has made a big difference with his FIC issue.

Just conscious about the last ultrasound that showed an enlarged liver and apparently the Mirtazapine can affect the liver. But it’s better he eats something I’ll likely give him a dose tomorrow just to kick start him again. Usually I give the Gabapentin early morning then wait an hour and give the Buprenorphine and odansetron together. Then I would give the Mirtazapine an hour or so later
 
Ondansetron can be given 3 times a day or sometimes 4 if needed. So why don’t you try giving it twice a day to start with to see if that helps. You may not need to give the Mirt.

Ah he’s already on odansetron twice a day he’s been on it for about three weeks like that. Maybe he might need it three times a day. How far apart does each dose need to be? Also would it need to be 1/4 tablet three times instead of 1/2 a tablet?
