Greasy coat: is this a symptom of diabetes? What to do?

I have been applying a foam shampoo every few days, yet his spine area is pretty greasy.
He also has a 'skin tag' about the size of a pea near the spine, and it seems to have a scabby top. I can touch it gently, so I don't think it's causing any pain. Could it be oozing grease? I hate to take him to have it removed - anesthesia etc. I have not seen him poking at it tho.

A greasy coat could be a symptom of many many things. Universally it means they are not grooming and that they feel extremely bad. When was the last vet check? It could have a lot of other reasons behind it. Is there any other symptoms the kitty is showing? We need more info to better help but my immediate recommendation is to take to the vet.

It could just be a simple skin bacterial infection and would not require removal of anything just some topical ointment or maybe some pills. Is it only on the back? I have a foster like that which is just a skin infection. It should resolve with medsif so but you will need the vet to give you some.
