Fructosamin almost normal but blood glokose level very high

Hi,

My cat is 11 years old, 6.2 kg (unfortunately obeze). We realized she is drinking too much water and took her to vet, our vet diagnosed diabete. He made blood and urine tests. There was very high glokoze (470) and high fruktosamin (1200) in her blood. But there is no keton in her urin and no damage in liver or kidney.
We started to use gripizid (2,5 mg twice in a day). After 12 days, her glokoz remained same but fructosamin level decreased to 400 (0.4). She has Amilin 1750, at the high limit. This results were not taken when she is hungry, so another vet recommends me to check these values when she is hungry (at least no food in last 6 hours). He also says pacreatitis therapy may also help to decrease blood glokose level (treatment with no sugar included serum and antibiotic).
I also measure her glukose from her ear blood and it is around 400 also at home. I can not undertsand how fruktosamin decreased ? I read from internet that fructosamin is used to differenciate diabetic cats from none diabetic cats beacaue it showed glokose level of the last 1-2 weeks .
Now we started to use 5mg glipizid twice in a day. We don't want to use Insulin beacuse my vet is hesitating a lot to cause any Hypoglycemic cases.
Meanwhile in 13 days, she lost 0.2 gr. weight which is good for us. We used hills W/D but now I changed to Royal Canin Diyabetic as this inlcuded more protein, lower carbonhydrat.

Please share with me any fructosamin experience you have with your diabetic cat.

I'm not sure where you are. Reading your post, I guess you speak another language. If you let us know what language, we can use Google Translate to be sure we understand each other correctly. Short sentences work best with this. I've started a table of translations here which may help.

American vets will use insulin at those glucose levels. The other drugs don't let the pancreas get better. The cat remains diabetic. The cat can die from this.

Because you home test, it can be safe to use insulin. If glucose is less than 200, no insulin is given.

These are good insulins to use - Lantus, Levemir, ProZinc, or PZI.

Food - high protein, low carbohydrate, moderate fat food - wet or raw. Binky's Page has lists.

I think the fructosamine test reflects blood glucose over the past 2-3 week, not 1 to 2 weeks. Glipizide does not really work in cats as your experience with unchanging blood glucose results has shown you. I can't offer you any reason why the fructosamine results would have changed.

On this board, we have had very successful results by feeding low-carb cat food and giving insulin. Congratulations on home-testing. That is the first step to getting your cat's diabetes under control.

Click on "User Control Panel" in the upper left corner of this page. After the panel comes up, click on "Profile." On that page, enter your city & country in "Location." Those people familiar with your location will be able to offer brand names for appropriate cat foods, etc.

Many thanks for your replies.
I stay in Istanbul,Turkey. I checked the list of 'Binky's Page' that you offered. Unfortunately, I couldn't find the proper canned foods (that include low carbohydrate, highprotein) in Istanbul. I use dry food 'Royal Canin Diabetic' for my cat. I hope it is not too bad, I know that it includes high protein but also 24-26% carbohydrat.

I talked with my vet this weekend. We will check my cats blood glokose and fructosamin level in laboratory next week. According to test results, we will start the inslulin. We can find both Levemir and Lantus in Turkey.

My concern is that, my cat is overstressed because I take her too frequently to vet. I think her glukose is also being effected from her stress. Maybe fructosamin test is giving more accurate results than blood glokose results. I will try to make my cat more comfortable and unstressed at home for a while and take her to vet afterwards for labratory tests.

Please let me know if you have any suggestions, many thanks in advance.

You need to start hometesting, because you are right if the fructosamine shows that he is pretty good regulated and the bg at the vet shows high numbers, then those are prertty much due to stress. The glipzide you are using have a lot of sideeffenct in cats and it is better to use actual insulin. But if you can actually find food with low card, maybe you dont need it. This food Felix, in danish called pussi, and in turkey maybe called something else has 5,5% of carbs. It has to be the as good as..... variants in jelly and only with meat and fish, not vegetablels. I think you should be able to get it in turkey. There is also the whiskas mmmmmmm variants with meat and fish, they are also pretty low in carbs. The dry food you are feeding your cat is way to high in carbs and you need to get rid of it as soon as you can find some low carb vet food.

Thanks for your comments. I think I can find some types of whiskas and feline in Istanbul. I don't want to start giving Insulin as my cat is alone at home all day. I am afraid of hypoglycemia cases. Let's see what will happpen when I change the food type. I will also check the rates of the canned food I find with the Binkys Page.

Gozde, I want to say welcome also - Isil and Hande will probably come and help you learn what the best foods are that you can find in Turkey! you want canned low carb food - less than 10% carbs. no dry food at all. it's best if you can change the food before you start insulin.

We can teach you how to test blood sugars at home. it sounds like you already have done some of that. it is the very best way to keep your kitty safe from hypoglycemia. these numbers are high enough that you will need insulin to keep kitty safe and help her return to being healthy.

so glad you found us. we can teach you how to help her. i'm sure Isil and Hande will help you also. They are wonderful women who have learned a lot about their cats and diabetes. They both use Lantus for their cats.

Please change her food to all canned, low carbohydrate food, no dry food at all as soon as you can. This could reduce her glucose 100 points in 1 or 2 days. Isil may have some suggestions.

Home glucose tests may be 100 points lower than at the vet. After she has been on low carbohydrate food for a few days, test her to see how she is doing. If she is 250 or higher most of the day, I would use insulin.

Insulin will work better than the pills. Insulin may let the pancrease heal and you may be able to stop using it after that. Pills will not do that.

To start insulin safely:
You start on a day when you will be home, so you can watch and test your cat and know she is safe.

You always test her glucose at home before you give insulin. This helps keep her safe, too.

You have supplies like honey or karo syrup in case she goes too low.

You should start with a small dose of insulin. This helps keep your cat safe.
Insulin is measured in units.
U-100 means there are 100 units of insulin in 1 milliliter.
For Lantus or Levemir, a small dose is 0.5 units to 1.0 units.
You give that dose every 12 hours for 3 to 7 days.
If you are worried about her, you can test in between the shots, too. A good time for a mid-day check is 6 hours after her shot. We call that +6.

After she has been on Lantus or Levemir for at least 3 days, you do a curve.
A curve is when you measure the glucose every 2 hours beginning before the shot, until before the next shot.

Based on the values from the curve, there are rules which say how to change the insulin dose. Changes are small, to keep the cat safe.
