First hypo BG 39 - BG 62 +20 - BG 77 +45

So we had our first hypo event about an hour and a half ago. Started walking around with her head down, like she was sniffing. Then walked a bit in circles. Tested and was at 39. Gave some syrup and some food and we’re going up. But what is a good number to see in this scenario to be comfortable? Over 100?
About to test again and I’ll update. Now at +1.25 we’re at 98

using an Alphatrak the number is 68. Good job with the syrup but keep in mind sugars can burn off quick and insulin can keep acting. Some medium carb food might not be a bad idea.

Ok thanks. Of course we’ve had issues the last 4-5 days with nausea and diarrhea, and she’s been picky eating. We’ve had to resort to the hills AD food, which is around 13% carbs, because she’s been so sick. But she did eat some fancy feast, well mostly the gravy. I’ll probably test again in an hour or so now that we’re up to 98

I would test in 30 mins personally but that's up to you
 
Much better. The AD is actually good for this scenario. 13% is medium carb. Did she eat some of it?

I don’t know if you saw my question on your last post. Did they test her for b12 deficiency and/or give her a b12 shot to be safe and rule that out as the cause of the lack of apetite?

I did ask if she was good on b12 and the answer was yes. Although, I’m looking at the blood test results and not sure what I’m looking for. Is it a specific name on the lab work?

I’m not an expert on lab results but the b12 test is a specific test and I believe it had to be done on an empty stomach.

David, as Ale has noted Coco needs a dose reduction but the last several days are not in the SS. To be able to help you we need to see the updated SS please.
The last test was the 137. I’m testing again soon as it’s almost dinner time.
I haven’t been updating the spreadsheet the last few days as I haven’t been testing as much. Unfortunately I’m getting a lot of information from different professionals and it’s becoming confusing. Plus the inside of her ears are red, crusty and sensitive from a reaction to the appetite stimulant gel. Also, lately when I prick the blood just smears and doesn’t pool up anymore, not sure why?

She’s been sick for almost a week now with diarrhea, some vomiting (not for 4 days now), nausea(I’m assuming) barely any appetite(some days only eating a couple tablespoons of wet food a day), and very lethargic. Mix in trying to manage the diabetes, it’s becoming extremely overwhelming. There was a moment yesterday where we were thinking it just may be her time as you can tell she's just been miserable, she is almost 17 years old. And to no offense to anyone, I’m not one to prolong the inevitable with feeding tubes or force feeding her or forcing her to sit in a hospital for days while they do exploratory medicine. She’s been miserable and mad with me since I’ve been pumping her full of medicine for days now, sometimes several times a days, and seen very little improvement. She’s seems to be making a small improvement today on appetite. She hasn’t pooped yet today to know if the diarrhea is better or not. Now, her diet is WAY outta wack. She doesn’t seem interested in eating her old hills kd food anymore. One vet wants me to change to the royal canin pr food for her digestive system but she doesn’t seem interested. The only food she seems to eat the last couple days is the hills ad. I feel its going to be tough getting her back to a normal diet, especially with what seems to be a sensitive digestive system. It’s likely going to take weeks and that’s if her stomach gets better and her appetite comes back. Plus I have to go back to work next week and not being home for 9+ hours a day it tough. Anyway but sorry I had to rant.

Bron, it looks like she got 2 units last night and 1 unit this morning so I wasn’t sure what the reduction should be…?
2u last night and 2 units this morning. I was told by the vet that if she eats even a little to give the 2u as he thought we needed to be aggressive with the diabetes as she is/was going down hill fast with her appetite and digestion issues. Now he wants me to drop it to 1u until we get her appetite and digestion issues back on track.
 
I’m sorry no one else has responded yet. How is Coco doing this morning? Sadly, what you went through yesterday is the exact reason we give dose adjustment advice based on how low the dose takes the cat in a cycle and not on the preshot numbers. Know that she could stay in high numbers today as she may be bouncing from the low drop yesterday.

I updated the ss as best as possible.
Today was the last day on the metronidazole.
Whatever light switch was off on her appetite has now been turned back on, she's actually eating as good as she has in weeks the last couple meals.
She had a normal bowel movement today for the first time in about a week.
For now we've stopped the nausea med and appetite stimulant.

Now I'm trying to transition her from the Hills AD food to the Royal Canin PR food that the vet recommended for her digestive issues. I'm having a hard time getting the dry matter facts for this food. I emailed Royal Canin but the last time I emailed them they said they don't have the dry matter facts, so we'll see. If anyone can help to figure this out, I would appreciate it.

She seems to have had a reaction to the appetite stimulant cream I was putting on her ears. One ear in particular (the other is not as bad) is really red and the inside skin is white/yellowish flaking. She did sit in the sunlight the other day for a bit so maybe its a reaction from that as well. Not really sure what to do, maybe coconut oil? Otherwise may need to make another visit to the vet if it doesn't get better. She's not messing with them or scratching them but you can tell they are a little sensitive as she doesn't let me touch them. Normally I can touch her ears easily.

The other thing that seems to be getting more frequent is she twitches when she is resting. I'm not talking about the twitch some cats will do in a deep sleep (she does this too) but it when she is trying to go to sleep or rest. She'll just involuntarily twitch her head or sometimes paws. It seemed to start just a few days after she was diagnosed with diabetes and was relatively subtle early on. But the last few days it seemed to get a little more frequent. When shes awake and up and about, she never does it, only when resting. She'll be sitting there with her eyes starting to close and then suddenly she'll twitch her head, sometimes once or even up to 3 times. Other times its like a slight muscle spasm or contraction. I feel bad because it seems to be tough for her to get comfortable sometimes. Vet says its possible its a neurological side effect of the diabetes. Can cats have this as a side effect of insulin? The other thing is she started on metronidazole and it was a few days later that I noticed this and I see this medicine says it can have neurological side effects. Now unfortunately she just finished the second round of this and now I've noticed it may have gotten a little worse, maybe just coincidence. But it definitely seems to be an involuntary movement.

When I was using the appetite stimulant daily & even now as I use it occasionally, I would always get a wash cloth warm & clean the appetite stimulant off of his ear every day before applying new. His ear would look a little pink/ red from it but that helped alot.
 
David I am glad CoCo is eating much better. However I would not stop the antinausea medications yet or the appetite stimulant. I would give them a few days more. And I wouldn’t rush off the Hills a/d if she is eating it and not wanting the other.

Can you take a video of the twitching and post it please so we can see it?
She shouldn’t twitch from the insulin, unless the BG is very low
 
I agree with Bron. I wouldn’t stop either. It could be that the metronidazole was affecting her apetite but I would keep her on the anti nausea if nothing else longer. Some cats need it daily forever. Which anti nausea is it? Cerenia or ondansetron?
