Dusty's AMPS 580 & Vet visit

Went by vets yesterday afternoon to purchase food and asked how soon I could have Dusty in for another
Urinary Analysis, Culture & Sensitivity. Dr. H asked me how long since the Convenia shot. I told her it would be 14 days
Friday. At first she just wanted to give me more Convenia. (I’m just appalled) I asked her if she would put him on a different antibiotic, that the Convenia didn't seem to be helping.
I looked back at my paperwork. Dusty has been on Convenia for an "underlying condition”, that they haven’t been able to diagnose, since February of this year. I was thinking March, but it was actually February. For the results of that particular visit, in the office I was told they couldn’t test his urine because his bladder was empty but they did test his blood. Those results showed his Kidney levels slightly elevated & his white blood cell count also elevated. That’s how the Convenia giving started.
I told her that I suspect that this UTI, if that’s what it really is, has been ongoing since then. I’m hoping it’s not a Kidney infection. Dr. H thought it would be good for me to bring him in on Friday (now today). I hope that the test won't be skewed by the Convenia.
I also let her know that Dusty peed in the litter box every hour during the night Wednesday night and that he pees a lot of volume every time. I told her his BG is still high and that he is on 6u now.
I told her I believe Dusty to be dehydrated even though drinking. His skin is tenting.
When I got home yesterday evening and tested, Yesterday's PMPS was 556. Dusty actually acted like he had more energy when I got home but Dusty did the same thing last night, peeing every hour during the night. I’m sure more dehydration.
Took him in to the vets this AM, he has become such a good car rider. It takes me 35 minutes to get him there. Vet H agreed about the dehydration and he was given subcutaneous fluid. She also took a sterile urine sample, with a needle, from his bladder. Vet H sent the sample out today with instructions to save some for a culture. She said she should have the results from the urinalysis back this afternoon and will phone me.
If the test comes back positive for bacteria in the sample then she will have them culture it to see what kind. She told me that doing it this way, of having them save some for a culture, that the culture wouldn’t cost me anything. I wonder if that’s a way of getting around working the lab, or if it was her way of extending no cost to me? She went ahead and prescribed and gave me Clavamox to give Dusty. I went ahead and started giving him the Clavamox as soon as I got home. No matter what, I know something is going on with Dusty although now I wonder if it’s not a UTI but a kidney infection. I hope not, but we may end up back in their office tomorrow morning. He may need more fluid. He drinks plenty, but it just goes right through him. Hopefully his kidneys are still functioning properly, and are just trying to clean his blood of all the extra glucose, but the dehydration shows me that his other organs are being depleted of fluid because of the over use.
Even after the vet visit, Dusty seems fairly normal. Although his sugar level is extremely HI, he still wants to eat and drink and use his litter box. He seems somewhat weak to me but he will sure tooten get up and ask for food when he thinks I need to put more down for him. I do think the urinalysis has caused him some pain. Since being home he has cried out a couple of times while walking around the house right before using his box. If he continues crying out I will call them and run back by there this afternoon and see if I can pick up some kind of pain medicine. I sure wish I would have thought about it while I was at the office. Personally I would of thought his vet would have thought about it but she didn’t. We’ll see.

Yes, paws crossed for some results.

The high numbers definitely play in his needing to drink and eat so much.
Scritches to poor Dusty as he goes thru the process of getting help.
 
A cystocentesis should not cause pain....if it does, they did something wrong.

I've dealt with chronic sterile interstitial cystitis (Gracie) and kidney infections (Gus) for years. I think Convenia has become a "convenient" way out for vets. They don't have to worry about whether you give the a/b or not. When it was very first introduced, my vet was gone and I saw another holistic vet in her practice and he gave Gracie the shot without asking me. She went into anaphylactic shock on the way home. I managed to revive her...while driving, get the car turned around, and get her back to the clinic. I had told him her cystitis was sterile....she never had an infection...which is generally the case with most, non-diabetic cats (she was not FD at the time). She didn't need an a/b.

Even though Convenia is more for skin infections, it is possible that it will have enough of an effect on the bladder bacteria that you will get a negative result. I'm glad they aren't charging you. Convenia also has been known to stay in the system much longer than 2 weeks....more like 3-4. I'm sorry to say that your vet has really mishandled this from day one.

Clavamox is a better choice if there is a UTI. However....for kidney infections, you might not see bacteria in the urine or in the culture. Gus has had a few kidney infections since dx with chronic kidney disease and his C&Ss always come back negative so the dx was done by ultrasound and that is what I'd recommend for Dusty. The treatment for kidney infection is 6 weeks of baytril.

If you can post his blood and urine lab results, I'd love to take a look. I've become pretty well versed in looking at labs having had three cats with CKD and Gus starting on year four with it.
 
I learn so much from all of you. :-D Marje, I don't have his lab results. I will have to ask for copies next time I'm at the vets. I agree with the mishandling of his case. It just infuriates me. I understand that I am Dusty's voice. I've been all my past baby's voice. I'm the one that pays close attention to him. What I don't get is how much I'm having to tell them and suggest to them. Anyway, I will get through this and am sooooo thankful for all of you with your vast knowledge and experience. I've felt really alone in this battle for Dusty and now I feel less alone with all of you pitching in and helping me learn. I want to post Dusty's mid day BG's. I can't believe I'm actually seeing some improvement since I upped his dose to 6u on 6/12. I could just cry for seeing this movement. At +7 it was 527, at +10 it was 473. We are getting there folks. We will reach his number soon I can feel it. I haven't heard from the vet yet regarding the results on the urinalysis. Keeping paws crossed. Oh, also Marje, I don't know anyone except for UGA that has an ultra sound, that drive is over 2hrs for me. I'll have to ask around. Surely one of the emergency vets would have one. There are a couple around that are about 45 min away. If it comes to that I may decide to go that way. Thank again for all the information. I'm absorbing everything I can from all of you for recall.

I'm sorry he is in pain....
It's very strange that he is painful after cycto...
I really hope he will feel better, and the lab result will give you some answers...

There are often vets who travel from practice to practice and provide ultrasounds. Also, if there is an emergency/24 hour vet hospital or a specialty practice, it's likely that they provide ultrasounds. I'm in Chicago an the nearest vet school is several hours away. There are groups here in the city that provide the services I've described. Hopefully, there is a means of getting an ultrasound, if needed, that won't require a long drive.
 
Yay for actual color on Dusty's SS. Progress! The fluids might have helped bring down the numbers as well. It's good to hear he's feeling a bit more active. My vet told me that a cystocentesis can make them want to pee as soon as they get home and I have noticed that with both my cats.

Depending on who your vet deals with for labs, they might be able to email the results directly to you. My vet deals with Idexx and it's pretty simple for them to redirect the results via email. Rather than you having to travel to the clinic again to get them, you can call and ask.
 
LOL, Dusty didn't wait to get home to pee, he peed all over the floor right at the vet.
Kelly, in at the reception desk was loudly telling Dusty, "you go boy, after all they did to you today they deserve to get peed on".
Dusty let his entire bladder go right there too. After watching Dusty all day, I think his pain is he bowels either that or his stomach is hurting after he eats.
It almost sounds like he's singing (not too loudly though) like when he gets ready to throw up.
Has not happened yet though but he had a small bowel movement earlier today and quit making the sounds.
Now late this evening he's started making them again. I can feel the poop in his stomach area. I really think he just needs to go do #2.
He didn't go at all yesterday. My thought is he's just full of poo again.
I have been giving him a teaspoon of pumpkin mixed in his food once every day and it has really made a difference with his poo, but the dehydration the past 2 days,
I think has also done a number on him. I'll hope for a good BM soon. Hoping that will ease his discomfort.
Dusty usually never makes any noise unless he's about to throw up so not really sure what this is about. Just guessing. He didn't start the singing until after he had the
clavomox. He has never had a problem with clavomox before, but it's been quite some time since he has been on it. I'm sure it could upset his stomach.

I never heard from the vet today so either she didn't get the results back like she thought or just never got around to phoning me. Maybe she will have them
tomorrow. I wouldn't be suprised if I end up phoning her before they close at 12:00 noon tomorrow. They are just so busy everyday. Usually its a couple
of hours after they have closed before I hear from them. I'm off to catch some zzzzzz's I don't know about Dusty, but I'm exhausted and really hoping he
doesn't keep me awake all night like the last 2. At least tomorrow is Saturday. If he will allow me, I may just sleep in tomorrow.
I'll be up at 6:00 a.m for his AMPS, Shot & food but I usually can go back to sleep fairly easy. We'll see. Good night everyone. Until tomoorow.
