Drastic Dose Time Change

I'm sorry friends that my kitty's SS is behind. I just got another job, and things have been a bit crazy. I have been able to maintain her on doses at 3:30am and 3:30pm of 1.15u. I have two questions.

I am cross-posting this from the Lantus forum because I didn't get enough help with the question on that forum. I know that Vicky can probably help me out here. Please reference that post for more information. http://www.felinediabetes.com/FDMB/viewtopic.php?f=9&t=58815

#1 if my time varies by 1/2 hour on either side, how should I vary my dose?

#2 MOST IMPORTANT: I am going out of town on tuesday/wednesday. Obviously I can't really ask anyone to do a 3:30am dose. My schedule for the next three days, including today, is as follows:
Sat: 7:00am-3:00pm, 4pm-11pm
Sun: 8:00am-2:30pm, 3pm-11pm
Mon: 8:00am-3:00pm, 4pm-9pm
Tues: out of town
Wed: out of town
Thurs: 8:00am-3:00pm, 4pm-9pm
Fri: 8:00am-3:00pm, 3:30pm-10:30pm

Can someone help me figure out how I am going to make this a decent time to ask my family/friends to help me out while i'm out of town, and then get back to the regularly scheduled shots?

You may not be aware, but we now have an Insulin Support Group for wonky schedules and unique dose techniques for Lantus - "Relaxed" Lantus. You can post there with questions like this instead of the Tight Regulation Lantus forum.

But Lev ISG was just fine. Nice to see you again and glad Baby is doing well.

#1. A half hour either way isn't too bad. Unless she's had a low recently, it's probably safe to change by a half hour once in a while. Changing the dose AND changing the time is a double whammy, so it's better to avoid doing both, unless the time change is more than a half hour.

# 2. You could give half at 3:30AM Tuesday then the other half at 7AM or whatever time is convenient for you that morning. That's a bit wider spread than I usually like for a split dose, but it would help her maintain lower BG than she would if you completely skip the 3:30AM shot and wait for the full dose at 7AM. Then whoever is coming would have shot times of 7AM/7PM. I would much rather you gave split doses to move the shot time to 7AM, rather than skip the 3:30AM shot as was suggested on Lantus forum.

I don't know when you get back Wed. night, but you should give a reduced dose at 7PM in order to be able to give a dose at 3:30AM. Maybe .75U, then only give .75U at 3:30AM as well, so she doesn't overfill the shed and go low. That's a 35% reduction, should be enough.

I hope this helps. I also suggest having your "Baby"-sitter give her only 1U, so there's no problem getting the right dose. It's not recommended to preload Lantus in syringes.

I don't know how you manage your work schedule. You have to be younger than I am! I certainly hope you have Christmas off. OH, you're subbing? Then I hope you have at least half days off next week! Bless you for helping Baby so well.
