What would you like to see in this to monitor the secondary signs of diabetes? Additions? Rating/scoring? Other?

The only other thing I can think of (for those who weigh their kitties) is 'weight'. But weighing will probably be an occasional thing and probably doesn't warrant a whole column to itself....

Much less (acting sullen, hiding, pouting, etc)
Less active (sleeping more, less engaged, etc)
Normal
More active (increased play, more social, etc)
Much more active (frantic, demanding, etc)

Change in gait i.e. difficulty walking, walking on hind legs, muscle weakness/twitching. How breath smells (i.e. foul, sweet-smelling, etc)

@Carl & Polly & Bob (GA) That is part of it. Plus, these can be good things to monitor in general as disturbances may indicate an issue.
