Dosing advice (too much insulin?)

Hi, I want to ask for your help again, I have been given my cat 1.25 u for a couple of days following the advice of increasing the dose by .25 but the last 3 days Nadir numbers have come up higher than before. In the Facebook group some people are telling me that my cat might be overdosing on insuline and that could be the reason numbers are coming higher than before, their advice is to lower the dose to .25 u and keep it for a few days, but I'm a little afraid to give him so little insuline, Can you please help me on this issue? Would greatly appreciate it. Thx.

I don’t know which Facebook group you are in but I totally disagree with them. There is no sign of low numbers that would indicate a bounce let alone too much insulin. Which method are you? Are you feeding any dry food? What are you feeding? It would be helpful to add that to your signature. How you had a chance to read the yellow stickies at the top of the Lantus forum on how depot insulin works and dosing methods? I think it will help clear this up for you.

It's what I understand, if it was a rebound for a high dose it would go from high to low numbers but I just don't see their numbers go down at any time. I have not filled in the blanks on the spreadsheet because I write them down with a pen and paper but they are never less than 400.
I am so confused and desperate! I worry about high numbers all the time.
The food is in my signature, it is Friskies pate. I have read the Lantus regulation method and I thought that maybe my cat would need an increase in dose, because after 3 days I do not see that the nadir drops a bit.

Could I ask why your cat was hospitalized recently? It is hard for me to see that your cat is overdosed, but the problem I do see, is the lack of tests, many preshots, and in part is that there aren't any night time tests, and only recently a few midcycle tests during the last few am cycles. What likely won't work is doing a rebound check and dropping the dose back down to 0.25u, like what is being suggested by your Facebook page friends. Usually that just ends up putting your kitty further behind, and allowing glucose toxicity to build up, making it harder to break through.

My suggestion is to test and see how the 1.25u is working before making any dose adjustments. I wouldn't drop the dose down, in fact your kitty may well need an increase soon if the tests show what I think they might, which is high numbers.

Hi Ceci!!

As I told you in the Facebook group, I disagree with the person who suggested you drop back to .25 and think you should hold the 1.25 for now and work on getting some of those missing test results plugged into your spreadsheet if you can.

I know it's hard to see them running that high, but cats do handle high numbers better than humans do. It takes a long time to do real damage.

It's too bad that your vet gave that shot of prednisilone but hopefully it'll work it's way out of his system soon and he'll start to come back down.

Seeing such high numbers after a prednisone injection for swollen gums, the vet suggested hospitalization to stabilize his blood glucose, prevent him from becoming dehydrated, and do a lab and US to see if there was another cause of the glucose spike. After we bring Mati home, the numbers are still just as high, but we rule out infections and pancreatitis.

Thank you Chris! I really hope to see those numbers go down, I'll stick with the 1.25 and do more tests, I'm a mess with the spreadsheet but I'll be more consistent

Oh poor Mati. I had been a bit worried it might have been diabetic ketoacidosis, we've had a few kitties here recently with post DKA. The labs came back showing no infection? It might be worth getting some Ketostix to test for ketones as well. Please do continue to post, we would be happy to help you get Mati into better numbers. If you have questions about the dosing methods that we use here, please feel free to ask.

I just wanted to make sure you were not feeding any dry food. Too bad about the prednisolone. So many end up diabetic from it. I think those cats are sensitive to it though as I had one on it for a very long time and she never became diabetic. Some after off it awhile go into remission. Fingers and paws crossed for you.

Thank you! Ketones were negative in lab tests, and we monitor as often as we can with urine strips. So far no ketones, thankfully
