DOSE ?Makoto switching to Lev- AMps( 285) PMps( 260)

HI there ! Long time - no post. I have been in Florida on a Covid medical travel assignment and Makoto was on cruise control a bit. After giving him time to settle in here and seeing that his BGs were still trending higher than I'd like, we decided to switch to Levemir. The internist, who I don't particularly like, suggested that we start at .5 units- since Makoto is on about 1 unit currently- but she said once a day to begin with. I just wanted to check in here and see what to expect with Lev and if that was a reasonable starting dose ( the 0.5 units / twice a day). I am supposed to work overnight tomorrow, so I was a little nervous about starting the new insulin on a night where I would not be home to monitor him, but my shift might be cancelled so...

Hi Ari, nice to see you! Wow on your assignment, but thank you for being in the front lines, I can't even imagine , glad you are home safe.

The switch from Lantus to Lev is 70% of current dose, so I'd say 0.5u bid is a good place to start. You may see an initial quick reaction to the switch, or not, Mav didn't seem to do that but I have seen others here with immediate reactions to the switch. Good luck and hope Makoto likes Lev a little better! Don't forget the onsets and nadirs can be later on Lev, so it is a bit of an adjustment.

HI we are actually still here. It's a 3 month assignment, so we will be here until the last week of July unless something happens.
I attributed his higher numbers at first to the move and adjusting but after sitting in the 300s mostly- I just decided to switch now instead of waiting until we get home.
Thank for the advice and I will make sure to keep an eye on the numbers at first and look for trends !!

Since you are on a fat 1.0 unit dose with not much greenage, I would go to 0.75 units. Twice a day of course. And you do want to monitor fairly closely the first couple cycles, just in case Makota is one of those kitties that likes to show a reaction. Neko's reaction was a solid blue second cycle. I fell in love with Lev, kicked myself for not switching sooner.
