Dosage Reduction?

Good evening everyone!

Marbles went from 70 at +7 today to 418 at +10. I'm thinking this was a bounce (as well as I think this morning's numbers may have been a bounce as well). Should I reduce her insulin tonight or keep it the same? I will monitor her around +2 and +5 tonight either way.

I am doing a BG curve tomorrow, so I hope to have more info tomorrow!!

Either way will give you info. Interesting that she hasn't bounced recently but did today. Wonder if she dropped really early? I think I would be tempted to lower a smidge for your curve.
 
Thanks Sue! I am going to give her .4 units tonight and tomorrow morning. I was hoping she was going to do well on the .5 units, but it seems too high for her (which I guess is a good thing that she needs less insulin). She has been eating more food than normal the past two days (2.5 cans versus 2 cans) as well which makes me think she is feeling the drop in BG levels and eating more to help compensate.
 
Interesting that I just tested her for her PSPM and she was 345. Definitely doing .40 and checking at +2. She is eating right now.
 
Any recommendation for her PM shot after the BG curve today? Do I keep her at .4 for the next couple cycles to gather more info (I go back to work tomorrow, so I won't be able to get numbers until +9, but could at least grab a +2 and +4 tonight as well as additional ones if she is too low)? I'm worried about switching her back to .5 because of the big bounce yesterday.

She hit her lowest around +4 today. I am a little worried that the insulin isn't lasting longer. Will this eventually change to a +6 and longer duration as her body adjusts to the insulin? Or do you think I should push the vet to change her to Lantus to give her a more even cycle? I wish I would have just started her on Lantus after seeing her bounce all around with ProZinc. I'm a little discouraged that I will ever figure this out today. I'm thinking ProZinc may not have been the best choice of insulin for her. I've also read about giving her the Prozinc every 8 hours, but that seems a little scary and not very manageable on my schedule as work would force a 9 hour gap.

Yes, I'd give .4 a few more cycles. The good news is that the insulin is working. It drops her into good numbers and she is below the renal threshold (250) for much of the cycles, so her pancreas can heal. You are giving a tiny dose and she is in good ranges.

The general recommendation is to give the insulin time. She didn't become diabetic in a few weeks; she may take a few months to be consistently regulated. I think you need to put on those patience pants :-D and keep collecting data.

It is possible that she would bounce less on Lantus. It is also possible that she would bounce like she does on PZI. Usually bouncy cats bounce until they quit.......
 
I knew the renal threshold was under 250 but didn't realize the pancreas can heal under that number. I was thinking she had to be in blues or greens for it to heal. That makes me feel better about her numbers. I gave her .4 units today and plan to continue that through the end of the week and getting as many +4s as possible. She is pretty angry with me today after all the poking - she put up a big fight for her PSPM test tonight and ran away from me when I tried to give her the shot, but she ate her treat! cat(2)_steam

Good to know that the insulin takes time and she will most likely bounce on any insulin. The vet made it sound like she should even out after 10 days. Glad to know that regulation can take a long time (even though I wish for a shorter time period).

Today was one of those days that overwhelmed me and makes me question if I've been making the best decisions for her treatment. Thanks for all the advice again!
