Doogle is home from the vet now

Ok just heard from the vet....all blood work was normal. He does have ketones but he expected that after being off on his insulin all week. His BUN is 72 and his urine fructosamine test was 400. The vet is going to send us home with b12, an anti-nausea and an appetite stimulant. He was dehydrated although he was drinking (it wasnt enough). He said it was hard to get blood since he was dehydrated. Pancreas, kidney and liver values all normal. So we are going to go pick him up in two hours. Leave his iv in and he will be sending us home with fluids (he said we are the only ones he trust to do this since my husband knows how to do it). He said he did perk up after getting some fluids at least. So anyone have a suggestions on an appetite stimulant I can ask for? He said what is usually given is a Valium derivative and he doesnt like keeping them on it for more than a few days. I am going to ask for all his lab work also. Is there anything I need to be aware of or ask before I got in an hour 1/2???

Hi Donna, they gave Lucky called Cyptolepadasme not sure of spelling vet writes bad it was for appy. hope Doogle will be fine

We give Zener cyproheptadine for appy stimulant, just a tiny crumb like 1/32 of a pill. We also give ondansetron for nausea, 1 mg BID. He also get cerenia, 1 mg 3 times per week. We think it helps Zener's appy and motility issues. Mostly helps with vomiting (get the injection if this is an issue) and nausea. We just started Zener on fluids and he feels LOTS better. Yeah remember reading many times that you use cyproheptadine. Is there a big difference between cyproheptadine and mirtazapine ? I dont think he will need the cerenia but I will type that up to have on hand in case I need to ask for it. I think the fluids are going to help more than anything. I have watched the video and read everything Marje typed up (even saved it as a pdf for future reference). I am just hoping all this helps and gets him back to a more normal Doogle.

We gave him mirtazapine a couple of times then he had a couple if episodes of head tremors and we stopped immediately. The cypro was originally intended as an asthma drug with a side effect of appetite stimulant. He has had mild asthma-like episodes so we don't mind the asthma treatment aspect of the cypro. The fluids will help Doogle feel better if he's a bit dehydrated.

He was dehydrated even though he was drinking water. I am hoping once I get the labs I will know more. Of course I will be back here asking all kinds of questions....LOL. Just hope I dont drive everyone crazy!

Cypro and mirtazepine are very different pharmacologically. Mirtazepine (Remeron) is a psychoactive drug that's commonly used to treat anxiety and/or depression. Cyproheptadine (Periactin) is an antihistamine and actually works in just the opposite way as mirtazepine. (Mirtazepine has a serotonin re-uptake inhibitor component whereas cypro is anti-serotonergic.) Cypro also has a much shorter duration.

Well we are home from the vet. The vet gave us something similar to Diazepam. I asked about the others and he said they are all based on valium. How long does it take to work? His gravity was .1035 and he told me the other numbers but I cant remember them all (we are getting the labs in the morning, their printer/copier is broke). He did say they were are a high normal. If he was concerned about crf or liver failure he would have done something. So we are taking Doogle back in the morning for more fluids and then they are sending us home with some. He wants to see how he is after being home. If he doesnt eat soon, things are going to get worse. We gave him the medicine about 2 1/2 hours ago. He just looks exhausted. It takes everything he has to just walk. If I get him some b12, what should I get and how do dose it? We have a multivitamin from when he got sick in May and it has 400mgs of B12 in it (plus other vitamins and such). If I cant get him to eat anything, how often should I syringe feed him? We have tried EVERYTHING (even dry food)!!!!
 
Yeah I joined the group and I am reading everything I can!!! I hope I wont have to do this for to long. Any idea on how long the appetite stimulant takes to work or does it take a couple times to start working?
 
The cypro takes about an hour to kick in. I don't know about Diazepam. There is a video on the FAF page that went through the whole syringe feeding process. Be sure to use a high calorie, low carb food that he doesn't eat normally. So that if he develops an aversion to that food, it won't be a problem when he is eating on his own.

Gotcha.....will do. Got any suggestions? He was eating FF classics (pates). Very very rarely will he eat Friskies Pates (only if he is in the other room with the rest of the herd). I bought some Tiki Cat and Merrick just because I know sometimes a new smell or flavor will interest him but it didnt.
 
Oh forgot to mention he did have ketones while at the vet but he thinks that is because he is so off on his insulin and no food.
 
What about getting a prescription for Max Cal?

Also, your vet is wrong. Mirtazepine is an SNRI (serotonin-norepinephrine reuptake inhibitor), cyproheptadine is an antihistamine, and diazepam is a benzodiazepine. They are entirely different classes of medication and work on different neurotransmitter systems. (And if he doesn't believe you, he can look it up on Wikipedia (let alone a pharmacology text). This is the section on appetite stimulants from Tanya's website for CKD. It's a good summary.

If your vet doesn't have a clue why Doogle isn't eating, did he suggest a consultation with an internal medicine vet?

If Doogle is developing ketones, it's essential that you get insulin into him. Obviously, that means food, as well. Ketones result from not enough insulin, not eating, and an infection/inflammation. Just out of curiosity, did the vet check Doogle's teeth. Is it possible that a dental issue is causing Doogle to not eat?
 
I take Diazepam for panic attacks, not sure why for appetite, I hope Doogle feeling a little better now
 
He is about the same really. He is lifting his head a little more to look at me or sounds. He wagged his tail some when I petted him. So thats better than it was earlier today.
 
I haven't had any experience with any of the appetite drugs, but I do know Valium is a tranquilizer. If Doogle is already sick, it seems to me that giving him a tranquilizer would make him even sleepier and less likely to eat...or do much of anything else.
