Hi! This is my first post to this group, though I have been following for a couple of months. All my previous posts have been in the Main Forum, maybe a couple in the Cansulin/Vetsulin group as that is what Disco was started on when first dx'd 10-25-2019. I am picking up a written rx from his vet today so I can order the Lantus pens from Marks Marine Pharmacy and she said she wanted to start at 1u BID but I know that someone said when I am ready to start, that I can get recommendations on dose here (I'd have to look back in my posts to see exactly what the message said). Anyway, I am anxious, nervous but ready to have his numbers be better than they have been for the past 7 weeks. His SS is up-to-date if you want to take a look. Thx in advance!

Typically we take the dose of the existing insulin into consideration when switching. Since he's been on 1.6 units of Vetsulin, you would do the same (1.5), or perhaps take a little bit off to 1.25 units if you want to be conservative. Switch when you can monitor closely the first couple of cycles. I presume you have the U100 syringes with half unit markings.

Yes, I have the U100 syringes with the half unit markings, started using them a week or so ago with the conversion chart from this group! Thx for the dosing info, too! I may at some point get a caliper, but for now will eyeball it from the 1.25 conservative dose to begin with.
 
Hey there and welcome to Lantus. I think you’ll be real happy with the switch. Since Lantus has a tried and true protocol to follow, it helps to take away the guess work. We switched from ProZinc and so happy we did!

Hello and welcome to the LB&L forum! I hope you'll get those calipers - fine dosing can really be a test ... hard to be consistent with my eyeballs anyway...

I use them every time I measure the dose...we're at 1.25 now - and I'd be lost without them.
