Did Sammy bounce?

Hi oh glargine gurus. It's only second day of this insulin, and I am wondering if Sammy did a bounce. He was 111 at +6 but then by 4:00 up to 354. He started off the day at 279. His asthma's been bad and he's had Flovent and albuterol today (different times). He seemed better at the middle of his cycle. I have to go put my horses in but will be back and welcome any advice after a glance at his SS. Thank you everyone!

Here's the link to your previous post here, for continuity: https://felinediabetes.com/FDMB/threads/sammy-in-the-middle.274616/

It's possible Sammy bounced from the 111. But it also takes 5-7 days for the initial Lantus depot to be built so numbers may wobble around due to that as well. Or it could be the asthma is impacting his numbers. Or my favourite reason "because cat!"
I was having a heart attack and so bummed out because first, I wasn't here to do a 2 hour check, and second, I was so happy he came down to normal numbers only to have THAT shock when I finally did his wellbeing stick. It is so frustrating. I worry that his asthma meds affect his blood levels. He's getting the Freestyle Libre 3 put in on Friday and I'm looking forward to not having to stick him for a few days (hours). Can't wait to be his buddy again (even if he DOES have to wear an ecollar). That bad boy's staying ON. At PMPS was down again to 315. Confusing but I'll take it. Gave 1 U. Ate well. Thank you for your help!

Which asthma meds does he take? We've had numerous asthma kitties here. Not so much the meds but the stress of an attack that can temporarily raise the numbers.

Instead of an ecollar, I've seen people do something like a onsie, or harness over the Libre. Or even a sock tied around the neck.

I think the coughs are attacks. At least they are for Pearl. I can't wait until you get the Aerokat and can start administering the Flovent with Phoenix able to take lots of breaths from it. I bet it will help a lot over time.
I am dealing with Sammy's asthma wheezing for days now. I am considering treating the Flovent inhaler like an oral, where you start off at the therapeutic dose then back off. He gets albuterol too. It's very tough. I do believe a recent post asthma attack reading was considerable higher rather quickly for glargine, and was guided to understand it's the stress of breathing difficulty. Makes perfect sense. I do so very much hope that the inhaled steroids aren't doing too much damage. Originally on Depo Medrol (Inj steroid) for years I'm pretty sure it caused his DM. But he definitely needs some. It's painful to watch them suffer.

the Flovent should be every 12 hours every single day. It’s the maintenance medication. It will not affect BG as an inhaler steroid. I’ve proven this multiple times with multiple cats blood sugars. The oral Prednisolone will affect BG. If he is wheezing then the inflammation is not controlled. That alone will cause higher BGs.

Once you get the Flovent it will take 2 weeks of use every 12 hours to reach full effectiveness. And you have to order it which may take weeks to arrive. So best estimate you are still a month away from Sammy getting onto the Flovent and it being full strength.

I would recommend oral Prednisolone until the Flovent is at full strength, unless there are conditions Sammy has that are contraindicated for Pred use. And I would recommend NOT tapering down the Flovent ever. The way the medication is designed to work is every 12 hours every day. If you decrease this, Sammy will be left with reduced or no inflammation support, which puts him at risk for respiratory distress and leaves the inflammation inside unchecked, also affecting increased BGs. Asthma is chronic inflammation, it’s not only about treating the attacks but keeping the underlying inflammation that no one ever sees properly maintained.

Albuterol won’t help much with daily maintenance. It is an emergency inhaler you give during attacks to stop them. Without a maintenance med on board (Pred or Flovent) you will notice the Albuterols effectiveness is greatly diminished or negligible.

I’m not a fan of the Depo shot. I prefer the Pred for many reasons.

Does this help? I am sick and going to rest now, so if you write back I won’t get it until tomorrow.
