Diabetes in Remission, High ALT, Mites?

Hi,

ChingChing has been in remission for a few months now, we are still home testing and keeping the spreadsheet updated.

She has had a liver problem in the past but for the past 3 months it has been back in the normal range, But today at the vet her ALT was HIGH, see spreadsheet.
She has been taking Hepaphos supplement, at first half a tab daily, then as her ALT has improved the vet has been reducing it to every 2 then 3 days. The vet has suggested taking more often again, giving for 2 day in a row incase the cut tablet is not working after being left open.

Also she has been grooming and licking a lot, the vet looked under the microscope and she has, I think Cheyletiella (he only told us in Thai so I think this is the English translation) a kind of mite.
He normally recommends Advocate, which is expensive here. Is there a recommended alternative, online I read Fipronil is the recommended treatment and here I can buy Frontline Plus which contains Fipronil.
We have 5 indoor cats so probably better to treat them all.

My main question is are these spot treatments ok for diabetes and liver issues? Any I need to avoid?

Is it better to treat ChingChing with Advocate and the other cats maybe with a cheaper brand?

As far as I am aware it is OK to use FrontlinePlus on diabetic cats but I have not used any of those treatments for many years as my cats are indoor cats and have not had any issues that needed treating.
I will tag @Suzanne & Darcy as she may know.
 
Hi there. I have some experience with mite overgrowth, although it was Demodex cati mites. My FIV+ cat had a serious overgrowth of mites last summer. We used Revolution Plus. I was not happy about putting any pesticides on my boy because I do not use them (cats are indoor) and I take even more caution with my FIV cats and senior cats. It took a few months because his infestation was pretty severe and his immune system was less able to control the mite overgrowth. We did skin cytology tests every two weeks at first and then monthly. Revolution Plus contains Selamectin which kills your type of mite. But there are other products and the insect growth regulator fipronil is also recommended. I’m not too sure about Frontline Plus use in cats. I would investigate people’s experiences with it.

As for elevated ALT, I have used Denamarin with great results. I see the 212 for the ALT, but what is the reference range for your lab? How elevated is it? Normally, vets don’t worry about ALT until it’s at least 3x normal. I don’t usually wait that long to start the Denamarin. It sounds like your liver supplement has similar ingredients.

  • Selamectin once a month for three months for cats, dogs and rabbits

From the vet I can buy:
Advocate Cat, 100 g/L Imidacloprid, 10 g/L Moxidectin
but its quite expensive, ok for 1 cat but a bit much if I use this one for all 5 cats.

Online I can buy:
FrontLine Plus Cat, Fipronil 9.8%, (S)-methoprene 11.8%
FrontGuard Plus Cat, Fipronil 9%, (S)-methoprene 10%, This is locally manufactured so is the best price by far, but maybe too cheap not sure I trust it?
Effipro Duo Cat, 50 mg Fipronil and 60 mg Pyriproxyfen
(Don't see Revolution Plus)

I am leaning towards Frontline Plus, but I do have concerns about giving it to ChingChing with High ALT and Diabetes in remission.

For the High ALT the normal range is given as 12-130, on the graph it shows only just into High range.
ALKP range is 14-111, so this is just inside the normal range

The article I linked said Imidacloprid was not effective. However, apparently the combination of Imidacloprid and Moxidectin together can be effective when used for 6-8 weeks. Fipronil is effective apparently. All cats should be treated, as you already know. It seems you have little choice but to put on the topical product. I would continue with the liver supplement for protection.

I also read that the Cheyletiella mites are zoonotic and can be transmitted to humans and cause “transient” pruritis and skin lesions.

Thanks, Yes I did read the article about Imidacloprid not being effective on its own, seems Fipronil is probably a better option.

As you say little choice really but to try it, so I have ordered Frontline Plus to treat all the cats. Although I am undecided if I should wait a bit until the after the next vets appointment and see if the high ALT was just a blip as it was ok the last 3 months and we have increased the liver the supplement, give it a chance to work a bit before using the Frontline. I haven't found anything negative about any of the treatments with liver of diabetes problems.
 
Is Ching uncomfortable? Itchy? Lesions on the skin?

She doesn't seem like she is very uncomfortable, she isn't scratching just licking more than usual. No lesions or bald patches.
I read that the symptoms are Cheyletiella mites are 'walking dandruff' but I don't see any signs of this.

Yes. I read that as well. Maybe it’s not a severe case and it will clear after on application of the Frontline Plus. Will your vet re-check it after a month or six weeks?
