Chloe - we are back!

My girl Chloe was initially diagnosed in 3/2021 and was in insulin about 8 months before her levels were well controlled enough to stop Lantus. She has been doing exceptionally well for over a year (nothing over 110) and then out of nowhere she jumped up to 300-400! Wow. So we just started Lantus 1/2 unit in the morning this week. She has bad arthritis for which she gets acupuncture weekly and Solensia injection monthly (adequen weekly). She does have pain often but it shouldn’t increase the level that much. She had I 131 in March. So we are back on this journey again. I’m sad that she has become diabetic again but thankful it can be treated and I can keep my baby around as long as possible. Thank you for your support.
 
Hi Mary, sorry to hear that Chloe is back on the juice. Did the vet check her teeth? Dental issues are the most common reasons for cats to fall out of remission.

What dosing protocol do you plan to follow? SLGS or TR? Is she eating a 100% low carb canned diet? No dry food?
When possible, please update the number of units you have given of Lantus in the SS?
 
Hi Mary. Our Tango recently came out of remission. Hopefully, both our kitties will rebound quickly:cat:

thanks Lori! Yes you’re right. I’m praying for that to occur. Have Tango’s levels improved so far, what is his regimen?

hi there! We are doing SLGS, currently at 1/2 unit. She’s eating only low carb fancy feast pate. Occasionally gets purina kitten canned which I think is low carb as well. She’s about 12.6 lbs and had got up to 13 something after I 131 so trying to get her back that missing 1/2 lb with high calorie kitten food! She gets a few pieces of dry on her bday and Christmas but otherwise none :)
The vet recently cleaned her teeth, I think it was end of February of this year. She was under anesthesia to do a test for her upcoming I 131 in March & her vet did it while she was out. So hopefully that took care of any dental issues.

@Bandit's Mom : is there something else I should ask the vet to look at as far as dental other than a cleaning? Let me know. She goes back Thursday for follow up :)
 
Tango's doing great. First diagnosed 4/22 - in remission 10/22 - out of remission 5/23. We went on vacation and when we got back is when his bs spiked. We think he was mad at us for leaving. This time around, he started dipping into blues and greens end of June. We skipped one dose and he went up a little. He does seem eager to go back into remission though. He only eats wet food. Before, his diet was mostly dry food. He adapted well to wet food. We were amazed. Tango has stage 3 KD but is not being treated for it. We do blood work every few months to monitor his kidney levels. He does have high blood pressure and is taking meds. Tango has a dental appt in a couple weeks. He has some inflammation, might need a couple teeth extracted and who knows what else. Hoping this helps with his diabetes.

Looks like you Chloe had her vet appt and going back Thursday. Hope it went well.

Glad Tango is doing so well! That’s encouraging to hear! So far Chloe has been 200s-300s but she just restarted on Sunday and only now at 1/2 unit. Previously she was on 1 unit so expecting we have to get back to that. I was trying to remember if I did lantus twice a day or once? It has been so long! Is Tango on lantus twice daily or just AM? I think Chloe’s vet is being cautious so we are starting very low.

We started at .5 and went up to 2.75. Now down to 2 and paw crossing the downward trend continues.

With Chloe just starting insulin it will probably take a little time to find the right dosage. Hopefully, it's soon!

Thank you for your insight! I was thinking I did it twice a day before. I guess her vet is just going slow. She’s still staying in the high 200s so I’m guessing we will bump up to 1 unit this week and I’ll try twice a day to see if it helps.

Vet called and said to increase the dose. So I’ll do that… she didn’t mention twice a day though I think it’s supposed to be. Should I increase it and start tonight? She was 260 at (+ 8) and 278 AMPS
I’m feeling overwhelmed. It’s been so long I forgot but doesn’t it usually take a few days to kick in?
 
Adding on to what Elise said, insulin should be given twice a day as close to 12 hours as possible. I'll defer to the others as to the starting dose since cute Chloe :cat: is out of remission.

I’m struggling with this out of remission thing, I feel like I did something wrong, so trying my hardest to get her controlled again and hopefully in remission again at some point!
 
Since you are going to the vet soon, I'd ask for a recheck of her thyroid and kidney numbers. There is a small percentage of I131 kitties who go hypothyroid. Thyroid numbers out of whack can impact insulin resistance.

I can't see Chloe's SS from 2021. What was her highest dose before going into remission? We might start her on a little higher dose (twice a day), depending on her previous dose - though I think she went into remission quickly when changing to dry food? We like to be a bit more aggressive with cats coming out of remission. The sooner you can get them regulated again, the better the changes of remission again.

I actually didn’t join this site till probably jan 2022, so previously I was only going off what her curves were at the vet. I have 2022 in my SS though. Her highest insulin dose was 2units twice a day and then she started getting low acting symptoms. She stopped dry food back in 2021 at diagnosis and that did make a good bit of difference. Probably dropped 40 points or so.
She actually just had her 90 day thyroid panel done and full labs at the end of June. I can post those in her SS too. I’m ok with being aggressive if it gets her back to remission. I think the vet was just being cautious.
 
Why don't you start with 0.5 units twice a day, and depending how she does, we might be able to fast track you up faster.

Thank you @Wendy&Neko! That sounds good. I’ll start this evening at 0.5 units. I don’t know if you had time to review but I did update most of her labs. I am missing a couple but for the most part it’s all there!
