I'm thinking either she needs more insulin or she needs less.
Early on, at 2 units, you got a green. You've not seen it since. Do you have a day when you could run a curve - test either every 2 hours or test every 3 hours (mini-curve)? That'd let you see the whole cycle of response and may provide some clues about what is going on.
