Changing from Canisulin to Levemir

Hello guys. So I'm changing my cats insulin but I'm unsure of the dose. Canisulin is a 40u/ml and levimar is 100u/ml. What would be the dose equivalent of levimar if he was getting 3 units of canisulin?
 
I'd start out with only 1u of Levemir at most. Caninsulin is a completely different dragon than Levemir so you're basically going to want to start over from scratch.
 
The dose is units, not ml; you measure out the units using a U-100 syringe for the Levemir.

Let me look at your spreadsheet; if you had really high numbers on Caninsulin at nadir, it may be appropriate to start a tad higher than 1 unit.
OK; I looked and the nadir tests are pretty high. You might take a point somewhere between 1 unit and where you are now at 2.5 units.

If it were Lantus (just another way to estimate a starting dose), you'd do the following:
How much does Oyster weigh?
How much should he weigh?
Take the lower weight.
Convert to kilograms if in pounds (pounds / 2.2)
Multiply by 0.25
Round down to the nearest 0.25.
And that would be for Lantus. That still gives us another estimate to consider.

Thank you for your help. Yes I know that the dose is in units that's why I mentioned he gets 3 units currently. I just didn't know how to knock back the dose since lev emir is a stronger concentration (100 verses 40) I ended up starting at 1.5. Thanks for looking at his chart and helping me decide. Really appreciate it!

One using of Levemir is supposed to be equivalent to 1 unit of Caninsulin although the volume of the one unit of Caninsulin is 2 1/2 time the volume of one unit of Caninsulin.
If you use the corresponding syringes; u40 for Caninsulin and U100 for Levemir one unit is one unit.
 
I have the same question for when I switch Nipper to Lantus. She's on 6U of Caninsulin now. I was thinking of starting with 2U and monitoring closely the first few days. From what my vet said, Caninsulin doses can be quite high compared to what I see people on here dosing with Lantus.

When you switch to Lantus it's like starting all over again. The TR protocol guidelines recommend calculating the starting dose based on ideal weight but then take into consideration previous response to other insulins. The first bit's science, but I'd say there's some artistry and experience required to assess the second part.

When Saoirse switched to Lantus, per the protocol she should have been on 1 IU Lantus BID as a starting dose, but because she had had such a strong response to Caninsulin our vet started her at 0.5IU. I worked up the dosing scale with increases at every 6 cycles until she reached the maximum she needed (0.75IU and not for very long - I had to drop the dose down 0.25IU again fairly quickly).
