I haven't been able to talk to the vet yet, but she emailed me the results.

(1) The urine culture came back negative so we're going to stop antibiotic this Friday (at 4 weeks).

(2) The thyroid level is normal.

(3) Cholesterol and ALP are elevated, which can be seen with Cushings disease.

Cushings seems like as good of a guess as any. Most of the symptoms are also just symptoms of diabetes, but the ones that jumped out at me were pot-bellied appearance, recurrent infection (e.g., urinary), and insulin-resistant diabetes.

At this point I think it would be helpful to you to inquire about both Cushing's and Acromegaly in the high dose ISG. There is a lot of info there, especially about Acromegaly, which is being diagnosed in more diabetic cats. Several of those members use Levemir for their cats, so they are knowledgeable about its use in cats who are diagnosed with these conditions.

If your vet suspects Cushings, you should probably have Celle tested for acromegaly too.
There is quite a bit of info from the Pet Diabetes Wiki on Cushings and several stickies in the High Dose group on both Cushings and Acromegaly.

I hope you can get this figured out. I'm wondering if maybe you should increase in .5U increments. What is the vet's plan for determining whether it's Cushing's or not?

Either condition is treatable and knowing what's going on will help you learn how to manage her insulin doses. Keep us posted!
