Cat gets lethargic after eating a small quantity of cat food pate hours after ProZinc insulin

Hello, my 19 year old neutered male cat was diagnosed with Diabetes 6 days ago. When he eats he gets lethargic, even small portions of FF pate and tuna steak. Also a bit spacey.

Background: August 30 he had a normal blood panel. Though his percisionPSL was a tad high but has been for years. He also had a depo shot. A week later he was drinking a ton of water and re-tested 9/16 resulting in a 675 glucose. He also had a fructosamine test resulting in 557. His glucose curve at 2 units went down as low as 296 but shot back up to over 400. Subsequently, he had his ProZinc increased to 3 units/once daily. Glucose after a couple days was 436 at 3 hours after insulin and 405 at 8 hours. He is and has been taking Elura for about one year. Only recently lost 7 ounces. Also, he has gone to the vet monthly for well over a year. Gets bloodwork at least every 3 months. Solensia monthly, etc. So not ignored. And most treats are healthy such as Fruitables.

Also, he had a birthday cake mid-August with tapioca and temptations salmon frosting. (this was not what I ordered). Regardless, it was a tiny cake, less 3 pieces, he licked all of the frosting but did not eat the cake in the course of 3 days.

He had issues with his hind limbs walking for a long time. This was originally thought due to having arthritis, stenosis.

Now he tries to use the litter but is not making it without my help. Pee pads are used last 24 hours. Can walk best in the early morning prior to insulin shot. Separating Elura and ProZinc out by several hours.

Now: He looks spacy after the insulin is given and after he eats. Looks fine after fluids, actually a bit more alert. Had been getting 50 ml of fluids/ twice weekly and now daily. He is dehydrated. He is drinking water too. Any idea about why he is getting spacey and lethargic. Under a vet care but no solid answers.

Are you testing blood glucose levels daily at home? If not, the spaciness and lethargy could be a symptom of hypoglycemia (low blood glucose level). Insulin can pack a punch especially a "big" dose of 3 units. It's important to test levels daily, just like it us for Human diabetics. Some vets discourage home testing for one reason or another but you can ignore the vet about that. Many newbies end up switching to a new vet who is more knowledgeable on treating feline diabetes or are just open to learning along with the owener.

The hind leg issue could be neuropathy. It's easily treated with methycobalamibin.

Thank you. He was tested at the vet for levels which were high over 400. The vet suggested that the lancets are hard to control the bleeding. He is too fragile for me to take to many vets, most of which would be long drives. Appreciate your insight.

ProZinc is a 12 hour insulin, he should be getting two shots, one in the morning one at night, and as Squeem3 says, home testing before each shot is especially at such high dose, are you feeding kibbles as well? My Corky is also on ProZinc, I will also tag Suzanne to help you with dosing, which seems to be quite high
