I shouldn't have to ask this.
Marco was abused for years before we got him and trying to pill him is an of futility. Someone else had this problem a few years ago and was given a YouTube link that actually worked.
He is not cheeking them, it's some kind of drama I've only seen once before.

Have you ever tried using a pill Popper - its a long handled grabber which grips the pill and then enables you to release it right at the back of the cat's mouth so it is harder for them to cheek or spit out & without the chomped on finger you would get if you tried it by hand.
I never used one but have seen them - there's a YouTube video on them somewhere.

Here it is

Another tip I have heard of is to crush the tablet, mix with a bit of water then pull it up to a syringe and syringe it into the cat's mouth. But I've never been sure because what if the tablet is supposed to dissolve gradually in the stomach but you are crushing it. Also you'd have to be very careful not to get it down the wrong tube into the airway - just like when the clever folks syringe feed I guess.
Anyway good luck with whatever you try!
PS I was sorry to hear about poor Marco's past, thank goodness he found a great home with you !

Thanks all. He's more than just resistant, he sees me as the Grim Reaper so it quickly becomes Marco between my knees, me behind him scruffing him head tilted back, rub his gums etc. In his mind this is it, I'm going to poison him.
His kidney issues are getting worse, he's 11 years old and who knows what those first six years did to him. He was also an outdoor cat.

is it a medication that can be compounded into a liquid? That might be easier.

All pill poppers are not created equal. The video Gill posted shows the this particular pill shooter. I swear by that type. I tried a few other without success. Neko made my up my pilling game. My time pilling shelter cats was nothing compared to her. :p One modification to the video - you do want to chase the pill with some water, so it doesn't get stuck down the throat. The pill popper I linked allows you to load a bit of water first, then add the pill to the tip, so you get pill shot and water in one go.

I'll try the water and pill gun. Maybe it's not the pilling but the aftereffect so the water might help there. He's 11 pounds and I have big hands so the pill gun would be less invasive.
I don't know what the med is but he can go in for shots but the vet prefers the pills. At least Marco doesn't make a fuss over the car and other people handling him.
Thank God for small miracles. Now I need a pee sample, that should be a hoot closing the door on four other cats.

I used to give Saoirse her meds just before her mini meals because the food would ensure the meds went down properly. I'd mix other supplements and certain meds into her grub.

From the time when Lúnasa became hyper-T I've not been able to find a wet food she can tolerate (uber-sensitive GI tract) and that initially posed a problem for admin of her meds and supplements. Now after I pill her, I give her a bowl of well-diluted cat soup (civvie, so no worries about the carb level). She really likes it and it helps wash the meds down without stress (she found it very distressing to have a water chaser given by pipette/syringe). I can add supplements to the soup as well. Both of us are happier with this solution. :)
