BG all over the place!

This is my first time posting in the PZI forum, I'm hoping maybe someone here can give some thoughts about what's going on with my little man's BG which has been visiting the 400s more than I care for despite higher insulin dosages.

Some background on my little man George: He was diagnosed in February and at the time also had Hepatic Lipidosis which required hospitalization. A few weeks after that he developed Ketoacidosis, again requiring hospitalization. We finally got him on track health-wise but the vets we were seeing kept raising his insulin dosage by whole units until he was at 5 units of PZI 2x a day. At the time we weren't checking BG at home and he eventually had 2 hypo seizures. Found this website that day and the people on the health forum said his dosage was way too high and I needed to find a different vet asap; I also began checking his BG at home after watching videos on this site. 2 days later found a cat-only clinic with knowledge about feline diabetes. They dropped him down to 1 unit PZI 2x a day and gave him a fructosamine test. The next day the test results came back and they said it looked like he was producing his own insulin and to stop giving him shots. I was thrilled...until a day later when his BG went back up so they eased him back in at .5 units 2x a day. Since then we've pretty much had to go up .5 units almost every week and are now at 2 units 2x a day.

He's been in the 400s quite a bit lately, I did a curve on Saturday and he went from the 400s to the 200s and back up to the 400s. I feel like even though we keep raising the insulin amount his numbers almost look worse. I've been checking for ketones in his urine and we seem to be ok there but I just don't know what to make of his numbers. I thought someone on the health board said at one point that if a cat is making it's own insulin and you give it more that can mess up their numbers, but I might have misunderstood, is that even a thing and if so could it be a possibility here?

His diet is primarily BFF canned food. He was a dry food junkie until a little over a month ago, I've tried feeding him Wellness, EVO, Fancy Feast and Newman's Own wet foods but BFF is the only one he will eat. I still have Purina DM dry around from before BFF and every now and then if he's being picky or I run out of wet food for a day I'll give him the DM, but he's mainly on BFF.

Oh and one last fun fact - his poop has been from the depths of hell lately. Loose and eye wateringly smelly. It started getting bad about 2 weeks ago and is just straight up disgusting now. I don't know if it's the change in diet from dry to wet? He's on his third day of Metronizadole pills to try and firm the stool back up *fingers majorly crossed* :roll:

So you think he lost weight recently, he seems off his food, has nasty poo issues, and some vomiting that doesn't seem to be hairball related? Has the vet checked for pancreatitis?

Others will be along for dosing advice soon, but worsening numbers after increasing insulin doses generally means too much insulin unless something else is going on to make numbers worse (infection, ptitis).

The vomiting seems to have stopped or at least become more of a rarity and I think he's beginning to put a little weight back on - not sure if the weight loss came from the stress of being boarded for 6 days? Is the nasty poo potentially a sign of pancreatitis on it's own?
 
Usually pancreatitis comes with other symptoms like loss of appetite and pain in the stomach/abdomen area, lethargy, just all around feeling like crap. Here's some info on the various things that cause diarrhea. Most times it is due to diet change or some sort of digestive tract infection. Those would be the two that are fairly easy to check.

Glad you made it over here! This forum is smaller than Health but we have all used or use PZI/ProZinc so hopefully we can help you and George. It is busiest in the am/pm East Coast time, so always check back for replies.

It sounds like you have a lot of variables. The dry food that he has access to can really mess with his numbers. Some cats need only a few pieces to increase numbers. I am wondering if FortiFlora might help. It's a probiotic (which might help with the poop issues) that is made with the animal digest that makes dry food so irrestible, so many people use it in the transition from dry to wet. You sprinkle it on top of the wet. If you could convert both your boys over to wet, that might help lower the numbers. It is available through some vets and on line.
 
Pancreatitis - you'll see high numbers, off food, often sitting in a meatloaf kind of position and could have tender abdomen.
My Shadoe had frequent issues, and normally you would treat with subQ fluids you can give at home, Pepcid AC-1/4 tab BID of the plain 10mg strength for the upset stomach, and some kind of pain med like buprenex because they DO feel pain but hide it well.
I was giving her Pepcid BID just a bit before ps times, and it would help her eat a bit in an hr or so. if she was still 'off' her food, I would give her a small amount of bupe. She would most times eat better after the bupe and pepcid settled in.

Because she had such frequent issues my vet had me start to give her B12 injections weekly to deal with the pancreas inflammation. About a month after starting her shots, she was much better and she had only one minor issue in the months that followed.

I can't comment on the insulin you are using as I've never used it for my cats - only caninsulin, lantus and levemir.

I would suggest that you test frequently for ketones and watch out for the bad combo of not enough food + not enough insulin + infection ongoing; you don't want to end up in the vet hospital again!

I should add about the food also.... my Shadoe was one who was eating wet but if she was able to steal even a few pieces of dry from a civie who would NOT convert, Shadoe's numbers would jump from 200 to 400 in a few hrs, and dry food stays the longest in the system, so you have a bit of a wait before it clears and the numbers come down.
You can also try freeze dried chicken on top of the food to eat, but I am pretty sure if you have an upset stomach on your hands, the pepcid will help a great deal.
