Best meds to treat nausea and combat pain?

Smoky is on Metoclopramide which is not doing anything for his nausea. He also may be having pain from flare ups from possible pancreatitis. He also has IBD and the vet thinks triaditis as well. Amy suggestions on good meds for him to take? I am taking him to vet tomorrow for bloodwork, urinalysis, fructosamine and spec fPL for pancreatitis.

Murphy has similar pathology (triaditis) - he takes daily cerenia (5 days on, 1 day off) which has helped tremendously - Murphy takes 4 mg daily but during a flare of pancreatitis or IBD, I up it to 8 mg daily. I would say this is the most important drug for Murphy. He also takes daily urosodiol and denamarin for liver support (but these may be secondary meds). When he has pancreatitis or just not eating much, I give him ondansteron - a human drug developed for patients on chemotherapy with nausea - for nausea and cyproheptadine- an antihistamine -for appetite stimulation. These latter two drugs are only given on an as needed basis. During pancreatitis flares, as Chris said, a pain med - bupe- is necessary as are subQ fluids.
Murphy is very sensitive to additives in wet food like carrageenan and the gums - these set off his IBD. So there are only a few brands of wet food he can eat. The best source of information for triaditis is ibdkitties.net ibdkitties also has a Facebook site where you can get advice.

My boy gets ondansetron (zofran) 3 mg 2-3x a day (always am and pm ) another dose if needed he is almost 15 pounds --when he was 12 pounds he got 2mg per dose
he also gets cerenia 1 a day (pm)
as said above if it is pancreatitis sq fluids and bupe are great (sq only if no heart condition)
 
If sub-q is a bridge too far then adding 3-4 tablespoons of water to each feed can help maintain hydration. (Warming the food slightly and blending the water well into the food can improve the amount consumed.)

Smoky started taking Denamarin Wed night for his liver. I will ask the vet tomorrow about him getting Bupe and Cerenia. I have been reading the materials on ibdkitties.net and there is a lot of good info. Smoky seems nauseous but has very little vomiting. I will check into the Zofran and urosodiol.
 
One word about denamarin - it says to give it on an empty stomach but a lot of cats (Murphy included) frequently vomit up the pill - it may be the blue coating. I have found that if he has a little bit of food on his stomach, he keeps the pill down.
 
Yep, Smoky is the same way with all pills. I have to give him about a half ounce of canned food first or a little bit of turkey. Does Denamarin have any bad side effects that you have noticed with Murphy.
 
I asked new vet about meds for nausea and pain. she said she can't prescribe anything until his test results come back in. She is extremely pushy and doesn't like you asking her any questions about the treatment for your cat. To me that's a " it's my way or the highway approach". I'll wait until I know what his test results are before going back there again

Ditto what I said on your other thread. This vet sounds like a PITA! All good vets prefer a collaborative approach with their clients. And only being knowledgeable doesn't necessarily make for a good vet - people skills are important.

I'd feel the same way as you, Lisa. Any competent vets should be confident in their treatment choices for their patients and therefore should have no problem with providing the reasoning upon which their treatment decisions are based. I'd be shopping for a new, collaborative practice ASAP.

FWIW if a cat is nauseated and can't eat enough it's very serious for any cat and doubly so for a diabetic feline. I think it's a wrong decision on the part of any vet to withhold nausea treatment until test results are received UNLESS the vet has a strong, evidence-based reason and can explain why to the caregiver.

BTW - I found ondansetron to be better overall than Cerenia for treating nausea. (Note: Saoirse didn't tend to have problems with vomiting.)

I'd feel the same way as you, Lisa. Any competent vets should be confident in their treatment choices for their patients and therefore should have no problem with providing the reasoning upon which their treatment decisions are based. I'd be shopping for a new, collaborative practice ASAP.
