Been a few days... 82 at pmps (skipped dose) and question

Okay.... I am so lost.... Harley appeared to be ok at 3 u twice a day w lantus. Then last week took a nosedive and while she was fine and didn’t show any emergency, we did give her stuff to be sure she was ok and monitored as you will see in the sheet.

since then, her numbers have been low. If I skip insulin for more than 24-36 hours, she goes back to 300s+ so surely she needs insulin of some range.
I’ve tried to work with a few doses we have discussed here and still have another night with no shot.

today we gave .25 .. and tonight we are low/not able to give a dose.
Today at midday she was 90.

We keep checking for ketones. None. Thank goodness. But am on high alert for them even w the low numbers.

I’ve noticed her appetite is changed. She licks her food and hasn’t eaten a full meal (2oz twice a day) in a few days. She may eat some -and I’ve added water to get her to come back to her food which helps but it’s not been consistent. I’ve been following her around to get her to eat anything possible and she comes out like she’s hungry but snubs her nose to stuff I’ve been trying.

Previous and for most of her life long before this diagnosis she has been a ravenous eater. The only other time she was a picky eater was when she had a severe pancreatitis about 10 years ago.

Tonight she ate some dry dr elesy and some greenies cat snacks but didn’t have much wet food. I’ve tried to bribe her with the ‘good stuff’ w gravy and she licks it but doesn’t eat much today.

She seemed to have a little loose stool /not fully formed and otherwise has been normal.

she comes out, bright eyes, purring, litter box otherwise regular, etc.

Just not sure what to do with the glucose reads and the appetite change.
Do we go to vet with “unexplained mild pickiness”. ?

I know fatty liver is a concern w inappetence and Harley did have a feeding tube w her severe pancreatitis so I’m no stranger to that rodeo if god forbid she needed it.

Ugh. I guess if we don’t get some at home answers soon, blood work to check liver enzymes isn’t a bad idea. Ugh.

anyways.... anyone have any suggestions? Do we let her ride out a few days of highs and monitor? Is it possible those 300s are some bounce I should ignore and try no insulin a few day’s and monitor? I will be a ketone strip nut if that’s the case more than now lol.

While I can’t help you with dosage, I can tell you that when a cat smells the food and licks her/his lips and won’t eat, it’s because of nausea. You should ask your vet about prescribing an anti nausea med like Cerenia or ondansetron. Minnie is on the latter and I give it to her, wait about 30 minutes and then she’ll finally eat for me.

Minnie is on Lantus too and while you can skip a dose for a cycle (12 hours) I don’t think is good to skip the insulin for days. Her depot will get empty and the numbers will go high again, but let’s see what others will say. Hang on! :)
 
You might want to ask the vet to do a test for pancreatitis. There are 2 tests. The snapfPL which will tell you yes or no for pancreatitis and can be done on the vet office. And the SpecfPL which has to be sent off site and will give you a number as to the severity of it.
I’m glad you are checking for ketones. Keep doing this.

With the dose you have been chopping and changing it. Lantus likes consistency.

Seeing you are skipping so many doses, I would try giving 0.1 units and see if you can give a dose both cycles. This may not be enough and you may need to go back up again.
