OK. so yesterday did not go well with his numbers in the 300s all day. I had given him fluids (about 75ml) mid-afternoon. He was a no-go on more fluids later in the day, so I gave him his B-12 shot last night. Between the fluids and the B-12, I guess that is what brought his number down this morning. I gave him 3 units last night, but with that 187 (and me going to work), i gave him 2.5 today. We'll see how he is when I get home.

I gave him 3 units tonight and he is outside relaxing on the enclosed porch after dinner with his brother...
 
I think you should be aggressive with his dosing because of his DKA history and trace ketones. Meaning, systematically raise him every 3-5 by 15-20% after getting a spot check in around his nadir time. Nadir is hard to pin down with him, but probably between +6 and +8. I would keep him food the same (add back that snack). If it's low carb, it isn't going to spike his BG and you want to make sure he is getting his calorie requirements. The recipe for DKA is high BG, infection and not enough food. And for some cats it takes only one of those conditions, not 2 or 3.

What can happen is that they develop insulin resistance and you have to keep raising the dose until they break through that. It's a response to glucose toxicity when their BG is above the renal threshold (~180-220), which he has been for a while. Don't worry about the pen. Lev will last to the last drop even if left out of the fridge for 24 hrs a few times. I'm sure you vet's treatment areas are air conditioned so it will be fine. Refrigeration is just get the longevity out of the pen. I use pens for months (like as much as 8) until it is gone and I have left them out of the fridge for a day or overnight. That's the beauty, and cost effectiveness of lev.
 
Thanks for the info, Shelia. He was a bit high today, so I gave him the 3 units and plan to give him some fluids tonight. I added his 2 snacks back and made them more like 1 tablespoon-ish versus the typical 1 1/2 that the scooper amounts to.
