Good afternoon,
unfortunately Binies urine test results came back positive. At the end of August she was positive with three different bacteria: Escherichia coli, Proteus mirabilis and Enterococcus sp. The problem was, that Binie can't take the most recommended antibiotic Amoxicillin. In the past she reacted on it after the first dose with vomiting and being very sick. And as Enterococcus sp. is a multi-resistent germ, there are not many options. We tried Pradofloxacin because it was available immediately while knowing, that Enerococcus reacts to it only intermediate. This means it may work with a high dose, what we gave her for three weeks.
In the new test we did after I finished the Pradofloxacin, the Escherichia coli and the Proteus mirabilis was gone, but the Entereococcus sp. is stil there in high quanitity.
The only other option, where Enterococcus sp reacts sensitive on and which is without serious side-effects for her kidneys, is Chloramphenicol. This is not an approved drug in Germany, so I did a research on how to find it outside Germany and I was successful. Unfortunately my vet didn't ordered it even though I asked her to and gave all the information and so we got the results yesterday and had nothing on hand to react. They ordered today (after several e-mails and phone calls from my side) and I asked them if I can give the Pracofloxacin, until the Chloramphenicol arrives. I don't know if it is a good idea but I don't want to leave her untreated until next week and my vet approved it.
Binie has horribly hig bg since days and today I saw only HI, when I came home in my lunchbreak and HI when I came home. I thought it was a bounce after the low numbers on sunday but for a bounce it is too long. So I hink it has to do with her infection. I know she usually is much lower on the handheld in red and black numbers but I am still very worried. Maybe it is also my fault. This morning before her shot her bg was going down and I thought she might be breaking a bounce. As I had to leave the house until 5.5 I panicked and gave her high carb (20%) for breakfast. I feel really horrible and have two more days where I have to leave the house for work. I hope we will manage, the pradofloxacin helps to reduce the Enterococcus and the chloramphenicol arrives fast.
I asked my vet if further diagnostic is needed and I like to discuss her answers here later this week. She is perfect in handling Binie, who is a difficult patient and she really helped Binie with many challenges so far. But I get more and more critical about her diagnostics and treatments.
Hope everyone has a relaxed day!
