8/4 Tibbs amps 368 need advice

Hi everyone
so i increased tibb's lantus last night to 1.5 units. I am doing a curve today and will post the results later. I know it will take a while before i see any results. my question is how long do i keep going with lantus before i look to changing to levimir? i am reading so many great things about levimir and i want tibbsy to get better as fast as he can. even though his numbers are high, he is acting normally, playing , eating but i am afraid that his high numbers are damaging his organs etc. should i be worried yet? and if not when should i start getting worried? i am so happy that he is obviously feeling better but i am still concerned about these high numbers. do we know what kind of damage is done by numbers in the 300's for so long?

I'm not sure there is a definitive answer. Anytime their blood sugar remains over 250 which is renal threshold you're running the risk of long term organ damage. But ecid. Autumn went 10+ months from when she was diagnosed to when I adopted her without any treatment at all and yet her organ function is perfect except her thyroid but it was damaged by an injury not diabetes.

Switching insulins depends again on the cat. I waited 9 months to switch Musette. Autumn I switched within about 3 weeks because of her tude and I had it available.

Personally and this is very much a personal opinion if one of my civies would become extra sweet I'd ask for Levemir from jump. I find it easier to draw, easier to decide up or down on dose, and easier on the cat.

My decision to wait was based on funds... I wanted to wait until I have used up most of my Lantus pens (ended up sending one to Michigan) before I switched.
 
ok. thanks so much. i do have a full vial of lantus so i guess i'll stick with it for a little while. hopefully the dose increase will bring him down.
 
No advice for you on switching since we have done well on Lantus, but in terms of changing the dose, it definitely does take the 3-5 days to fully take effect like people say. We only had one real dose reduction because of how fast Mona plummeted after switching her to wet food, but based on my extensive spreadsheet stalking, it's interesting to see the consistent, gradual shift cats have after dosing changes. But, I know how it feels to want it to happen quickly to help them NOW!
 
thanks. it's good to know that the lantus is working well for you. he has come up more in the low 300's mostly so i'm hoping the 1.5 will do a little more.

Hi Nadine,
It sounds like Tibbs' story is just the same as Noodle's. His AMPS today was 361 and when I spoke to the vet today he said he wants me to increase the Lantus to 1.5 also. He seemed to think there should have been a sizable decrease in Noodle's BG after the two weeks on Lantus which isn't always the case from what I've read on here. I broached the subject about switching Noodle to Levemir but the vet wants me to at least try 1.5 U for 4 days. He seemed lukewarm on the Levemir just because he's "never used it". S-I-G-H. I live in a tiny town and have already run through the other vet so don't know what to do. I hope you see some real progress with Tibbs soon. Believe me, I know how frustrating and worrying this is, as Noodle's numbers are still in the 300s also and I fret about long term damage while we wait for the Lantus to kick in, if it ever does with your Tibbs and my Noodle. Thinking of you--hang in there!

thanks christi. it does sound like we're in exactly the same place. i just went up to 1.5 yesterday so i am hoping to see something in a few days. all the stories i have read about levemir seem like that's the way to go. i am waiting for my vet to get back to me on it. i hope we have success with lantus. i will keep looking for your posts to see how ur doing. good luck. we can keep our fingers and paws crossed together. :-D nailbite_smile :-D

just wanted to tell you that i spoke with my vet and he also said he doesn't have that much experience with levemir either. he did say tho that if tibbs doesn't respond to lantus that we could try levemir so we'll see how it goes.
