Tonight will be the 6th cycle for Scully at the .75 U dose. If I don't see green tonight, I'm increasing her back up to 1 U. She successfully held the reduction to .75 U in July, but it's not looking good so far now. We're still not seeing any improvement in her neuropathy, and it's going on 5 months now that she's been on insulin and Zobaline. However, as her numbers show, she's not well-regulated yet, so I guess that's why we're not seeing any improvement. (sigh)

Mulder has done a complete 180-degree turn-around from Saturday! I am sooo happy to report that he grazed all day yesterday (I counted six meals!) and throughout the night (DH said he ate twice before he came to bed.) Today when I got home from work, I found both bowls just about empty. I let Mulder outside for about 15 minutes, then when he came in he sat where he eats (I was washing the bowls at the time) and demanded dinner, loudly. I was very happy to oblige!

I picked up both Mulder and Scully's lab results from the vet. Our vet uses a local lab called Clinilab in Valparaiso, IN. Here are his numbers:

Whoops! Got a little trigger-happy and submitted before I was finished. Sorry! Anyway, creatinine is high, but BUN is within the normal range. Also flagged for being high are "K" and "OSMO." NA/K is flagged for being low. Also, just to let you know, a urinalysis has not been done. Any and all information you can provide is much appreciated. Thank you so much for looking over Mulder's lab results!! Even though he's not a sugar kitty, I really am so grateful for all of the help and advice you all have given us. Every day I continue to be thankful for finding this board!

Those labs aren't bad at all. I would wait a couple of weeks and then check the creatinine and get a urinalysis. A part of the possible CKD puzzle is the USG (urine specific gravity). If the USG is even on the low side of normal it can mean kidney disease but if it is well concentrated not necessarily. When Tiffany was diagnosed she was symptom free. I brought her in for a dental and when I picked her up found out her teeth were scaled but she was not sedated because her creatinine was high. I had to bring her back for a recheck before my vet would diagnose. I don't know what OSMO is. Do you? The phos is fine and you don't need a phos binder at this time in my opinion. What are you feeding him?
 
When Scully was diagnosed with FD, I switched both cats from an all-dry diet to all-canned Fancy Feast (turkey, chicken and beef.) My vet just recommended we switch him to prescription food (Hills k/d.) Yesterday I gave him a can of Friskies Special Diet Turkey & Giblets and he and Scully both like it. If I need to switch him to a low-phos food, I'd prefer it to be one that Scully can eat, too.

We looked up OSMO, and found that it is a measurement of the balance of water and the concentration of all chemicals found in the fluid part of the blood. Both Mulder and Scully's OSMO numbers were above the normal range, so one thing it seems to indicate is dehydration, but I don't really know much more about it.
 
Glad Mulder is better!! osmo is osmolality and tells whether the blood is concentrated or dilute. It can vary based on dehydration, diabetes, CKD, and other things. It can be high from dehydration or from acute kidney disease to name only two things.

Did the report day if the sample was hemolyzed? His potassium is high which can be a concern. His Na/K is off because his K is so high. Several things can cause high potassium such as AKD, trauma to tissue.
 
Marje, you are a wealth of information!! Where on the report would it say if it was hemolyzed? It says "Type Serum" on the page with the results. Did I mention he was born with one kidney smaller than the other? Could this affect the numbers? Prior to the test, he hadn't eaten much in over 4 days, so although our vet didn't think fluids were necessary, I would think he was at least partially dehydrated. His coat also has some dandruff, and since he has begun eating again, he also is drinking more water. Before last week of not eating, I never noticed him drinking out of the bowl. In the last few days, he is drinking out of the bowl and out of the faucet in our bathroom sink. Simple dehydration from not eating or kidney problem?
 
Usually if there is a number that doesn't fit and it's abnormal, if the sample is hemolyzed, it will state it and put + if it's just a little hemolyzed or +++ if it's a lot. Usually hemolyzed samples occur if the kitty struggles a bit or if the sample is not handled correctly after the blood is drawn.

I would take in a urine sample, as Elise suggested, and have his USG checked as a baseline. I'd try to get the first urine of the day as it will likely be the most concentrated. Cats' USG can vary slot during the day and could be dilute at certain times. It's important to recheck it a few times if the results are dilute. You don't want to base anything on one test.

Conditions other than kidney issues can cause them to drink a lot of water and then you would expect to see dilute urine.
Did they check his T4? I didn't see it on the labs.
 
Hmmm....

If he were mine, I'd do the urine. I'd also make sure there is no infection anywhere. I'd recheck labs in a month or so. I'd discuss with with your vet the timeframe considering some of his numbers and how he drinks out of the faucet, etc.
