Ok, here we goooooo! Cycles 13 and 14 on drop dose today, steady in GREEN numbers. Pearl's BGs on this drop dose attempt have been fantastic - not a single blue aside from her Pred cycles. This is much different than the last time we got down to the drop dose and she hovered mostly in blues.

So tomorrow dare I think it's time?! (I have a hard time even writing this for fear of jinxing). Can people please advise - assuming we hold steady the rest of today, are we cleared for OTJ trial (eeeek!) or am I to do something different for awhile more, stay on this dose etc?

Yes ma’am! Looks like our Pearlie might be ready!! Congratulations! And, if you could please, modify the subject line since we’ve responded. Thank you!

  • Start the trial on the next green pre shot.
  • If she is green at your normal test times, no need to test further until the next "PS" time; just feed small meals and go about your day. If she is blue at your normal "PS", feed a small meal and test again after about 3 or 4 hours. If her BG is lower 3-4 hours after a meal, then the pancreas is working!
  • Post every day so we can monitor her progress and see if any tweaks are needed. She may have a sporadic blue number. Don't panic but post before you decide whether to shoot so we can have a discussion.
  • After 14 days of no insulin, we have a party!!
Sometimes the trial doesn't work the first time and we have to give a little more support in the form of resuming insulin. It's not the end of the world if that happens; we just give her the support needed. Our goal is a strong remission and it's better to take our time to get that than to rush into remission just to have it fail later on.

Once she is through the trial successfully, you enter a new phase. Pearl is still diabetic but has now become diet-controlled. Continue feeding low carb food in the manner successful for your kitty. If you decide to change her feeding schedule, let your meter be your guide to the best times to feed. Avoid medications with sugar in them and steroid medications unless they are medically essential. Continue testing blood glucose weekly for the first month and then monthly forever. It's a good idea to weigh her monthly. Weight should remain stable. If she seems "off" or sick, or is showing signs of diabetes (excessive drinking, eating, urinating, weight loss), test her blood glucose right away. Keep the teeth and gums clean and healthy; dental issues can bring a cat out of remission. If you see rising blood glucose numbers, it's time for a visit to the vet!

Eeeeee, so very excited but trying not to be, lol! No matter if the trials works or not at this attempt - it is such a FEAT Pearl has gotten to this stage - I have not dared to hope. I'm so thankful we are here and so hoping her pancreas cooperates.

You know...I have already noticed that her BG is lower 3ish hours after eating, going on for the past week or so. I thought it was weird and didn't know how to word it in my posts here, so I'm glad you mentioned that part.

I will post daily and I will start her trial tomorrow morning (or tonight if she looks pretty green).

Thank you so much for all your support and cheering us on for so long, for helping me moderate my expectations with my Lyme disease, and I know you are as excited as I am to see how far Pearl has come!
 
Also one more question! @tiffmaxee @Marje and Gracie I have been thinking about trying to reduce Pearls current Pred dose from 2.5mgs every 4th day to 1.25mgs every 4th day since her asthma is so stable. Do you recommend I try this now during the OTJ trial or wait until afterwards, so that I’m not varying multiple things at the same time?

I would do it. If anything it will make her numbers better. If the asthma gets worse you would just take the dose back up. I get not wanting to change more than one thing at a time. See what Marje thinks.

No pressure on you and Pearl at all . . . remember it's called a trial (just to ease the pressure). I agree entirely that Pearl looks ready for a reduction this time:):):):):)
 
Suer exciting! This is a real testament to you, Margaret. You deserve all the credit for managing her multiple health conditions and bringing her to this point! Paws crossed for success!
 
Fantastic news! And a great example of what can be done even if kitty is on pred. Well done and good luck with the trial. :D
