8/15 Tibby: Doing a curve after a bounce

OK, so Tibby had a big bounce after I skipped a dose because his AMPS number was low. Now it's back up where it was. I'm assuming I should wait another 7-10 days before I do a curve and adjust his dose, as if it were a fresh start?

I had just upped his dosage from 2.5 to 3 units, and the next day is when he had an insanely low AMPS. So I freaked and skipped, and he bounced back up to a 500 number in the day, and now after a few days he's back in the 300s before shots.

Hi Amy,

Have you read the stickie (at the top of this forum, with the yellow tabs) for SLGS?
https://www.felinediabetes.com/FDMB...-low-go-slow-slgs-tight-regulation-tr.210110/

I'm not sure why you reduced - I didn't see a number that was low enough to have earned a reduction - unless I missed something that was discussed that day?

I'm also not sure why you skipped when he was at a nice medium blue number - have you been advised not to shoot below 200? Although I saw that you did shoot 3 units when he was at 100.

I guess I'm kinda confused about your dose changes...but I'm kinda busy during the week and often miss conversations...nothing in the comments section for me to go on.

So, first I'd recommend you read the information in the dosing method - SLGS - and then change your dose based on those recommendations. It will help the depot to build and his numbers will gradually become more reasonable.

Hope that's helpful...

I have read it. That's why I skipped, because I read the part about, if you don't know how your cat reacts, maybe skip. His number was blue, but my vet advised not shooting below 200, and that sticky cautions if they are below 150, so I just figured better safe than sorry. Then I pretty much did a curve just to make sure his numbers rose. The time I shot when he was at 100 was before I knew much of anything. That's the day I started the spreadsheet, because it freaked me out. That's when the vet said after the fact that he probably would have skipped the dose.

I've read everything, but do I need to hold the dose from the day I skipped, as if I had changed the dose? It seems like the bounce maybe threw him off his rhythm, and I should hold this dose for the 7-10 days before I adjust his dose again. I just wonder if any nadir would be reliable when he had such a bounce recently.

tagging @Briere Fur Mom, @tiffmaxee for help with TR dosing methods...

Typically we hold the dose for a minimum of 6 cycles/3 days before increasing unless a reduction is earned..

It’s late for me so I’ve tagged some folks who may be able to explain in more detail when to increase or decrease your dose and /or skip...

I'll come back and explain SLGS and TR. Someone else may get to you before I get back. Nonetheless, gotta cook supper:banghead::banghead::banghead:. Will be AFK for a lil bit;)

That's okay. I'm doing SLGS. I was trying to increase his dose when the drop/skip/bounce happened, and I lowered his dose back down because of it. I'm just wondering how long I need to wait before I do a curve and think about raising the dose again, if needed.

You should have increased to 2.75 units, not 3.0 units. How about trying 2.75 instead? Maybe he won't get those blue preshots you aren't comfy with yet if you are on that dose. You don't need to wait, increase now. At 2.5 units he needed an increase, so he still does. Actually, the numbers on 3.0 you'd seen meant he didn't earn a reduction either but if you are doing SLGS you should go there 0.25 units at a time.

That sounds good. I knew that SLGS would mean an increase of .25 to 2.75, but the vet wanted him at 3, so I went with the vet. I'll start 2.75 tomorrow. Thank you!
