? 8/13 Boris Input needed

Well, so far Boris has been very stable on 0.1u... we suspect he had a hairball one day (and I panicked and gave extra insulin), and last Friday he had his FeLV immunization, which drove his BG numbers up a bit.

Overall though, this dose has treated him very well.

Although he has earned another reduction according to SLGS, I haven't done so yet. We can try a drop dose again, but I definitely do NOT think he's ready for an OTJ trial. When I look at other SS where cats have gone into remission, their numbers have been much more in the greens... Boris has been in the high greens/low blues.

Do I try a reduction back to a drop dose, or is he ready for an OTJ trail and I'm just not seeing it?

ETA: Or do we hold here and call him controlled and stable?

It looks like Jackson has come down nicely since the start of his SS, so you too may face this conundrum in the future! ;)

I have to say, when we were at the vet on Friday, the nurse kept talking about cats they treat who are in remission on the prescription diets, and she asked what we were doing. I don't think she knew what to make of the low carb wet food making such an impact on his BG!!!

I keep waffling on whether to stay where we are or go back down to a drop dose. I'm just hoping somebody will chime in with extra information on how to make that decision (beyond the SLGS protocol).

You don't have to take the reduction. Boris's numbers are pretty good right now, but the PS numbers could be lower. Is there any way you can feed him during the middle of each cycle to try and flatten him out some and hope his PS numbers come down?

We currently feed every 6 hours, using a timed feeder overnight. At shot times he gets 1/3 of a 5.5oz can, and at +6 times he gets 1/3 of a 3oz can. Any suggestions on what to do differently? Those PS numbers really are what is giving me pause...
 
One other option is to feed Boris more often -- the same amount of food just broken out over the cycle. As a cat's pancreas heals, when you feed your kitty, his pancreas produces its own insulin as a means of getting glucose (the by product of food being metabolized) into the cells. This causes BG to drop. If you were to feed Boris at +3 and +9 in addition to at reshot times, it might help to lower the preshot numbers, which are typically your high points. It may be worth giving this a try for a couple of weeks and see if he responds.

That sounds reasonable for us... we'll give that a try!
 
You know, in thinking about this more, maybe we just need to spread the calories out more evenly across meals instead? Right now they get 1/3 of a 5oz can at Boris's shot times, and 1/3 of a 1oz can at +6... maybe that's too much of a fluctuation in calories throughout the day?
