Just got home an hour ago. Mary is very happy to be home.

I've got to give her antibiotics, Pepcid AC and sub Q fluids at home and take her back for a recheck and labs in a week as long as everything goes well at home. Venita has been posting updates with more details about her stay at the vets in her DCIN blog that you can link to in our signature.

The vets were doing curves and her nadir was varying between 6-12 hours there. So they want me to dose the Lantus at 1u in the morning and 0.5 units in the evening until we can get a better idea of her readings at home.

I don't get out of LL much and didn't even know Mary was hospitalized for DKA. I'm so sorry but I'm happy to hear she's stable and back home.

Just a question regarding the vet's recommendation... Do you know if they were also giving Mary R (Humulin R)? It's a very common approach with DKA to have a dextrose drip going and to give a basal insulin (like Lantus or even Humulin N) and a bolus insulin, like R. If they were doing basal/bolus dosing, you're probably not going to see a clear nadir since the R is a short acting insulin and it can give you a very different looking curve.

At this point I would follow the vet's recommendation but be aware that you are most likely going to see wonky numbers. Remember, Lantus likes consistency. It may be worth talking to the vet about splitting the difference and giving 0.75u twice a day.

I know this may seem obvious, but please test for ketones as often as you can. Just because Mary's numbers may look good, it doesn't preclude ketones from developing. Did the figure out what the infection was?

They used the Regular insulin for the first couple of days and then switched her back to Lantus. I'll keep a close eye on her numbers and keytones and once I've got a few days without the vet stress and infection we should be able to confirm the nadir and adjust the dosage. They are not certain but suspect the infection was a UTI though a culture came back negative.
