Today marks the fourth cycle of Dak being on 6.0 units of Lantus, and he is doing well (still a food gremlin and biting our shins or calves if we don't hurry up with preparing his food ha!) and it seems that his ketones are starting to stay in the lower range (for him at least based on the past month).

Would there be a reason for his BG value to basically plateau for the whole day today? Since we are at day number 4 of giving Dak 6.0 units of insulin, should we go ahead and up his dose to 6.5 units tomorrow morning?

With yellow nadirs, you can and should increase every 3 days/6 cycles. I count 4 days/8 cycles on this 6.0 units.

As for being high and flat, it just means "high and flat". I remember one member used to call it the pink pancake or a trip to IHOP. :p

Now that Dak has gotten to 6.0 units, you might want to consider asking the vet to run a couple blood tests for secondary endocrine conditions that can cause a cat to need more insulin. The two I'd start with are the IGF-1 test that tests for acromegaly, and IAA (insulin auto antibodies). My cat had both conditions. The blood for both tests goes to Michigan State University. Acromegaly is from a benign tumour on the pituitary that causes excess growth hormone to be produced - think teenage growing boys and their appetites. One in four diabetic cats has this condition according to fairly recent research (2015 paper). There are treatments for it. IAA is sort of like an allergic reaction to the injected insulin that can go away in time.
