? 6/10 Zeke - BW Labs Attached - Advice? Thoughts?

We're still waiting on all the tests results from Friday but the vet called today with some initial results. Test showed that Zeke's fructosamine at 283. Vet said that this number is an indication that Zeke is very well regulated as a non-diabetic cat would test anywhere between 140 and 450. I didn't realize that the fructosamine would not correlate with the BG readings taken with our Relion. Is this the case?

As the test represents his avg for the past 2-3 weeks, his high numbers during the week of May 26 through June 2 are driving the average up so I guess we can assume that his fructosamine would have been better while we were working towards his trial.

Our objective for requesting the fructosamine was to provide reassurance and peace of mind that our glucometer has been accurately reflecting his BGs. Do you guys agree that the 283 is a good number (and reflects very good regulation) and also that the 283 seems to be inline with the BG on his ss?

Yes that's a great number! The fructosamine test scale is very different than our glucometer numbers are and that's considered excellent control. I don't have the link to the scale right now but congratulations!

Thanks Chris for the quick response. Well, that's one less thing to worry about. I initially :eek::eek::eek: when he said 283.

The vet is recommending that we get an ultrasound of Zeke's abdomen to see if there are any GI related issues. We're working on getting that scheduled as soon as possible.
 
As much as I dislike the insulin, the Caninsulin wetsite lists some Fructosamine levels and their meaning. However, this paper seems to differ somewhat on what is considered a normal level. In either case, Zeke is very close to the reference range. :):)

The fructosamine does and doesn't correlate to BG. The scale that my vet used was:
  • 191 - 349 = normal,
  • 400 - 450 good control,
  • 500 - 650 fair control,
  • > 600 poor control
The lab running the test may use a different scale. Fructosamine is similar to a HgA1c test for humans. A result of 6 would be comparable to a BG of 135 which would be a mildly elevated BG. (Normal HgA1c is considered 4 - 5.9)

From my perspective and from a statistical perspective, an average is nice. However, knowing what the variability is along with the average is even better. One outlier can throw an average off and you can get the same average result with wild swings as you can with numbers that are very consistent. As long as your meter is reliable, I think you're in far better shape looking at your day-to-day test data than relying on a fructosamine test.
 
I initially thought that BGs corresponded to the Fructosamine too. Fortunately, those high Fructosamine test numbers aren't equal to high BGs. Zeke looks good!

ETA: Sorry, I just looked at his SS (I keep wanting to call it a score sheet). I didn't realize he was back on.

I now have all the labs from Zeke's visit on Friday. Looks like maybe mild pancreatitis, although Zeke did not fast prior to the test, so I'm not sure how much an impact that's had on the results. That morning the vet had told us it wouldn't make much difference if he'd already eaten. Do you agree the labs supports pancreatitis? Do you see anything else of concern?

He's scheduled for an ultrasound on Friday, June 19th at 9:00am. Although the appointment is at 9:00am they'll probably have him for a few hours and may not start the ultrasound (I'm guessing) within 30 minutes to an hour after he arrives. Intially, I was instructed to have him fast from midnight on. However, when I explained that he was getting insulin, I was told I could give him the insulin and "feed him a little". I normally feed him 3 times after his shot, 1 tablespoon plus 1 teaspoon of food each time. His shot is at 7:00 and feedings normally go until 8:30 or 9:00. So do you think I should give him insulin that morning and if so how much food should I give him? I don't want him full and they will most likely sedate him, but I'll have to confirm he won't be anesthetize.

I would ask the vet how much you can feed. What percent of the meal is "a little"? When Neko had her dental, I was told I could give 1/3 of her food. You could also decide how much insulin based on his preshot that morning. You may decide a smaller dose is appropriate if he's not getting his full meal until later.
