5/23 Simon AMPS 312 back from vet

Simon was just dropped off at vet for x-rays and possible U/S. He had declined over the last few days. BW didn't show anything that unusual except for high amylase. He is always a little high so no major concern. He doesn't want to eat but did eat some HC food I gave him. I also syringed fed him. For those of you that know Simon, he has always been a big eater. I have also noticed that his left jaw looks slack and he is drooling somewhat. He had also thrown up a stringy vomit into his food dish a few times. Simon is lethargic and just wants to lie under my bed. He also makes a little noise in his abdomen when I push on it and pick him up.

I gave him some subq fluids last night and during it, Simon started making this huffing coughing sound. This also happened at vet on Monday when they gave him fluids. I am so scared.

Please send lots of healing vines to Mr. Simon.

(((Georgia))) so sorry to hear all this about Simon declining. I'm glad he will have an U/S. I remember the fluids and coughing incident at the Vet last time. Many prayers and healing vines for you and Simon. I hope a diagnosis can be confirmed and treatment started. Keep us posted when you can, Georgia. Lots of hugs and love from us.

Many prayers and beautiful healing vines going out for Simon this morning. I so hope that the vet can make a diagnosis. Has Simon had the spec fPL test for pancreatitis? (it has to be sent out to IDEXX labs, but many vets can do a "snap fPL" in house and that would at least tell you whether pancreatitis is present). Pancreatitis is very painful and could be the cause of Simon's distress.

Vet just called. Abdominal and chest x-rays look ok. Vet is not ruling out pancreatitis and is running test today. Ella, I didn't know about a snap test and will ask the vet about it when they call back. Vet says Simon is dehydrated and is recommending IV fluids and hospitalization. I don't want to leave him there overnight because they do not have staff at the clinic overnight. I don't want him to stay at an ER because he is an extremely nervous kitty and will not do well there with a bunch of dogs in the same room. The vet feels ok with doing IV fluids while I am at work today and then me picking him up at 6:00 and bringing him home. I will bring him back to the clinic tomorrow. Vet does think there is something wrong with the GI system and pancreatitis may definitely be the cause. Cancer is still not out of the question, but I will need to do an U/S to find out anything else.

Question: should I waiit until the pancreatitis results are in before I do an U/S? Or should I go ahead and do it? I hate to think about cost, but I just don't have the money to do all these things if it is not necessary. DH is self-employed and he has not been able to work much because he was dealing with his father's illness and death, and now, taking care of his mom's issues. :sad:

Sending many, many prayers for Simon and for you and DH. And his mom.

While the best way to dx pancreatitis is a combo of the specfPL and U/S, if you are worried about the costs, then just go with the test. I wrote a Primer on Pancreatitis which might help you with info to discuss with the vet.
The snap test is just a yes or no; the test they send off actually gives a value. It's best to fast before the test but if he's that sick, he has probably eaten very little.

The only advantage to the U/S at this point, if money is tight, is that it will let you know a little better if you are worried about cancer. It still might not be definitive. IMHO, I'd wait for the results of the test (just be sure they are doing the correct one...there is an old one that is not as accurate). If it indicates he has pancreatitis, I'd address that now and wouldn't worry about the U/S unless he doesn't improve.

Are they going to start treating for P-Titus, even though you don't have the results back? It sounds like sweet Simon could use some pain meds as well as anit-nausea meds, too. I haven't dealt with P-Titus in a VERY long time, but maybe you should ask about starting meds.

On the flip side of Marje's response, I think there are a couple additional factors to consider. If your vet is going to send blood out for a spec fPLI, it will take a couple of days to get results. (The Snap fPLI is done in the vet's office and the results are immediate. It's like a home pregnancy test type result.) With p'titis testing, there's a window for when you get the most accurate results. If you test too early or too late in the bout of p'titis, you may get a false negative.

With an US, providing they have a good sonographer, you can have immediate results.

If you're bringing him home tonight, if he's still eating well, ask the vet for anti-nausea meds (ondansatron/Zofran or Cerenia/maripitant) and maybe once those meds kick in, an appetite stimulant. Food is going to be essential to not have liver complications and to keep ketones away.

Marge, thank you for the pancreatitis primer. What great info and I'm going to look at it before I go pick up Simon.

I spoke to vet and he is going to go ahead and do the snap pancreatitis test today. I figure it at least will give me an idea of whether it is pancreatitis and then we will do the specialized test to see what the actual number is if positive.

I am definitely going to ask for more pain meds. I had some bupe left over from Cleo and gave it to Simon yesterday. He has already started on pepcid and ondansetron and had one round of Cerenia on Monday. I will pick up an appetite stimulant from vet today, too. I really didn't notice a big change with the meds, but hopefully, he will improve and want to eat. This is so not like him and it is making me so nervous.

I am still trying to decide what to do about the U/S.

Georgia, I'm sending tons of get well vines for Simon. I hope this is something easily remedied. I know how you feel about getting nervous when he doesn't want to eat. Champ is also a big eater and when his appetite is off, I KNOW something isn't right. I hope this all gets sorted out sooner rather than later for your peace of mind. Hugs to you both!!!

Simon is home and doing pretty well. Snap pancreatitis test was positive so it looks like it is pancreatitis. Other test is bring sent out for more specific results. He ate some on his own at vet and then has eaten more when he got home. I was given bupe for pain and he got a cerenia shot and Pepcid and had IV fluids. He will go back tomorrow morning for more IV fluids and I decided to do the U/S if I can get more credit on my care credit card ): I just gave more pain med. Hoping that it is just pancreatitis and nothing else. I will let y'all know about U/S results when I get them back tomorrow. Thanks so much for all your thoughts and prayers.
 
I'm glad you know what you are dealing with and Simon seems to feel better. I hope each day finds him improving and that the U/S will be negative although if he has pancreatitis, they should see some changes on the U/S.

Whew! When I first read that Simon was ill I looked for your post & was glad to see that Simon has been Dx'd..
Sorry it's that Lousy P'titis, but at least you know & can treat him & he will soon feel better--

I'm sorry that it's p-titus, but glad you have a diagnosis and a plan...and really glad that he seems to be feeling better tonight. I hope you guys are able to just be together and relax this weekend!!
 
Morning Georgia - sorry for the DX, but at least now you have a plan, and can work on getting Simon better. Healing thought and vines for him. I hope he recovers quickly.
