Good morning,

Yesterday's condo

Yesterday
AMPS=135 (0.75u); +2=184; +8.25=148
PMPS=178 (0.75u); +2=160; +4=302; +6=284; +7.5=304

Today
AMPS=335 (0.5u BCS, not eating); +2=295; +7.75=176; +9=184
PMPS=184 (0.75u); +2=209; +4.5=151; +6=133; +7.25=218

Lucy's been sick since she vomited last night at around +4.
I gave her Cerenia, but I think it came out with the 2nd vomit.
I gave her about 100ml of sub-q fluids because she wasn't drinking much after the vomit.
And since she seemed uncomfortable throughout the night, I gave her Bupe about 3 hours ago.

She's still feeling sick, and not eating.
Since I didn't want her to stay so high, I gave her a BCS, 0.5u instead of her regular 0.75u.
She wasn't tightly regulated on 0.75u anyway, so I thought 0.5u would be safe even if she doesn't eat.
I hope I thought right.

I do have to leave home during day today.
I'll call the vet later to see if I can get an evening appointment.

Thank goodness you got an appointment for this afternoon. Maybe you should ask for injectable cerenia to have on hand and a script for ondansetron. It might be a good idea to ask if you could have given her more cerenia last night as it's my understanding that all of it is not absorbed anyhow so a little extra won't hurt. I'm sending prayers for your vet to help her and Lucy to feel better quickly.

I've offered many different foods, and she doesn't want any of them......

It doesn't really seem like her usual pancreatitis flare for some reason.
Maybe because she does't seem painful.... She just seems sick.
I'm so afraid what if it's her kidneys failing.

Mariko, just a thought. How long has Lucy been taking Ursodiol? If it has been added recently, I wonder if that is causing some nausea? :YMHUG:

Lucy's been on ursodiol for many many years, close to 10 years
So I don't think that's it.....

I have to leave soon, and won't be back for maybe 5 hours or so.
At least she can sleep without worrying about me doing stuff to her.

Also, this is just a thought without knowing too much about Lucy's meds history....do you think she might benefit from having Pepcid and cerenia on a regular schedule? My vetty is a big fan of this combo. She wants Trix to stay on pepcid BID now, since the CKD can give them an acid tummy that can snowball into big ups and downs. My vet also thinks it is OK to give one dose of cerenia every other day. We're holding off on that for Trix at the moment only because she seems to be doing OK without it and because she seems hypersensitive to meds in combination with her BP meds.

But I wonder if something like that may help keep Lucy on an even keel. It's something worth investigating.....

It's so hard to leave when they aren't feeling well.

I just got back.
She seems to be feeling better, and she ate a little bit.

I don't want to stress her any more, but I should probably keep the 4:30 appointment.
I hope the stress won't take away the little appetite she regained.

She had a part solid part pudding poo just now, which is good so I can bring that to the vet too.

We are back from the vet.
She got a shot of cerenia, a shot of famotidine. and a dose of cypro.
Even though I have cypro at home, I asked the vet to give it to her there so I don't have to struggle with her at home.
We are also getting some b/w and urinalysis done, and will hear the results tomorrow.
I also got metronidazole for her pudding poo, to start from tonight.

I got a new box of cerenia and some bupe to keep at hand.
I asked about the injectable cerenia, and the vet said he can prefill some syringes for me but they won't last long.
So he suggested to call and pick up when I want it, so it will be fresh.
I didn't ask for a vial of cerenia because it will probably cost like $150, and a shelf life is only 28 days.

I FINALLY got a Rx for ondansetron!
I saw a different vet today, whom neither of my cats had seen, because Lucy's usual vet was not working today, and the 2nd vet was in surgery today.
I knew the doctor Lucy saw today is also very good and detailed, but just by chance we had never made an appointment with him before today.
He thought cerenia was better, but gave me the script anyway so I have a plan B if something like last night happens again.
He said ondansetron in tablets might not be as wildly available in Canada ("might be available only in US"?), so he suggested I call around.
He also thought that it would probably have been OK if I gave her the 2nd dose of cerenia last night even if I wasn't sure whether she vomited the pill because cancer patients get much higher dose, so if she get more than her regular dose one time, we wouldn't have been in much trouble.

I think that's it.
Almost time for her PM shot and dinner.
I'll post how it goes.

She ate the 1st dinner well.
The meds are working. I am so relieved.

It sounds like you got everything that you need. It is true I think that ondansetron is hard to get in Canada. You want the generic. I think for that reason and when vets don't use it much, they hesitate to prescribe it. Both on the FPS and lymphoma lists we have found that some cats do better with cerenia, some ondansetron, and some need both at times. Also, some need more of it than others. Max is one of those. I hope Lucy feels better and you don't need it. I'm glad you vet agrees that you could have given an extra dose of cerenia. That will help in the future if necessary as well.

What dose of metronidazole were you told to give? That is a drug that is better to start of the lower end and the dosing parameters are large. My vet was telling me to give the same amount to Max that my friend gave to her 30 pound dog. It turned out I didn't need it but I wasn't going to give that much after discussing it with the vet on the lymphoma list.

About metronidazole, I was told to give one 50mg tablet every 12 hours.
Lucy has been on metronidazole a few times in the past, and I checked the previous empty bottles, and the dose has always been the same.
He wanted me to try for 5 days first, but gave me enough for 15 days in case a longer course of treatment is necessary.

Does this metronidazole dose sounds OK?

ETA: She weights a little over 5kg, so around 11 pounds.

Mariko, I'm so glad you got a vet that was so thorough and listened to you, and even happier to hear that Lucy is already feeling better! I'm sure you'll sleep better tonight knowing that.

I'm really glad you got I to the vet and got her some meds for her to be more comfortable.

If you can order from thrivingpets.com, they have vials of injectable ondansetron for $19. You just have to fax or email them the prescription. I ordered it and received it in just a couple days. I just like to have it on hand.

I agree with Elise that sometimes one or the other works better for specific cats. I've often used it in combo.

I was relieved to hear that Lucy is eating and feeling better. Yeah for medications that work and a good vet visit. :-D Not a bad PMPS for a BCS shot this morning. More appy vines coming your way in case you need them.

I just looked up thrivingpets.com and they don't ship prescription drugs outside of the US. :sad: But that's what I expected.

I just looked up the dosing of metronizadole and the low end is 7.5mg per kg. Max is almost 15 pounds and his dose was 50mg. So Lucy is getting a little more than the low end but not a high dose. I feel better knowing she has been on it before without issues. On high doses it can cause neurological problems but Lucy is not getting a high dose. We think one of the cats on the FPS list had a reaction to it a couple weeks ago. I definitely got the feeling it happens from high doses and longer term. I think you are fine. When I got it for Max he only had the problem for two days, once each day. I was going to start it the next day if he still had the loose stool but he was normal the next day .

I'm glad the meds are working!!

The vet also really wanted me to give Lucy some hairball remedy.
The only hairball remedy Lucy accepts is Vetoquinol Vitalax.
I hadn't given it to her for a while because I suspect there might be some sugar in it and also she doesn't groom as much anymore.
But the vet said that accumulated hairball can really make her uncomfortable and really wanted me to give it to her and even gave me a free tube.
So I gave it to her tonight. She enjoyed it very much.
I thought that, at the moment, making her tummy feel better is more important than micro-managing her BG.

He also wanted me to give her another sub-q fluids tonight, so I did that too.

Phew.... I gave everything I needed to give her today.
Lucy is doing good. She's happy.
I'm exhausted today, and ready for bed.
I'll get up to test at least once before AMPS since I gave her fluids when her number is dropping.
Hopefully no drama at PM time tomorrow morning.

Wow, from 133 to 218 in 1.25hr after the hairball remedy.
So Vitalax must have sugar in it... probably like giving her karo. :?

Hi Mariko, I was getting caught up on Lucy. Sounds like a really good and thorough checkup with this Vet. Glad Lucy is feeling better. Sending hugs and continuing healing vines. Paws crossed for good b/w results.

I don't know if you saw Marje's note. Ondansatron is available as an injection and it has a long shelf life. The injectible is a lot less expensive (at least in the US) than the tablets.
