He looks good, Kathleen, so I think it depends on how much you want to push, and how often you can monitor. He definitely has been below the renal threshold the last few cycles, which is excellent. But he does seem to have room to drop, as long as you are around to bring him up with food if he needs it.

So maybe one unit on anything over 200. (he hasn't been going over 350 in a while. If he did, 1.2). If between 180-200 maybe .8. What do you think?

Remember to have the plan for a dip into the 50s in mind (because if you do, he won't go there :mrgreen: ). 50s, feed a few tbs of regular/medium carb food and retest in 30 minutes. 40s, high carb gravy and retest in 20 minutes. Below 40, high carb maybe mixed with honey and retest in 20 minutes. Keep offering small amounts of food and retesting until you have three rising numbers.

It's a little scary to push for green nadirs, but it is the path OTJ.
 
Okay....going to make a chart and put on my fridge. Not sure I can be around everyday to push for those low numbers due to part-time job. But will try on off days.

Is there some point when the pan crease heals and just starts producing enough - after all this time? Or is the damage too extensive to expect OTJ? I don't want to get my hopes up unrealistically.

I think it falls under that darn "every cat is different" thing, Kathleen. There certainly have been cats on insulin for years who eventually go OTJ. Check out Bookworm's comment on the PZI Roll Call this am. Lisa has been working on it for several years and is giving less and less insulin while Cassie stays in better and better numbers.

All you can do is try, and meanwhile he is in nice safe numbers and feeling good.
 
Great job getting the numbers over the weekend. I'm going to offer up my 2 cents now.

Look at your numbers analytically. Here's where you've gone on the following doses since April 5:

1.2u... Range=334-379
334 incr to 379 PS
370 drop to 238 PS
379 drop to 131 Nadir/PS


1.0u... R=238-345
238 drop to 128 PS
255 incr to 370 PS
257 flat to 222 PS
268 flat to 255 PS
299 drop to 233
321 drop to 177 (+9)
344 flat to 334 PS

0.8u... R=165-263
165 flat to 154 (+3) 263 PS
199 incr to 268 PS
222 flat to 199 N/PS
242 drop to 95 (+8) 165 PS
263 drop to 131 (+6)

0.4u
198 incr to 242 at PS

128 incr to 198 PS

Hopefully I haven't missed any. The skip and .4 I think were bust. Safe if you can't be around, but otherwise, bust. There was overlap in the range of numbers you've shot a 1.2 or 1.0u dose into, and though you came down most of the time, it was never into scary low numbers, just one nice high green. I would like to see you shooting a more consistent dose from one shot to the next. If you're above 250, I would stay with 1u for now. Actually I would be inclined to say over 200, but that would ONLY be if you'll be around to check. Keep the dose consistent for at least 3-4 days (8 cycles) and see if you even out. If you don't, then I might consider that sliding scale to be 1.2u if over 325 or 350.

Heather was here for a while, and her Sneakers needed a couple days to settle into a dose. I'm thinking that may be the issue Fred is having too, and by changing the dose frequently, he's not settling in, which is why you're seeing the ups and downs you're seeing.

Thanks for all that work, Lu-An. My vet also recommends sticking with a number for a while. I give all this some thought and come up with a consistent plan for this week, then I'll post the daily results for all of you to see - and advise me on.

It's so good to have you all out there to help!!
