30/7 Angie ambg=90 - day 14 no juice on our "not-otj-trial"

Good morning all,

Angie had been a bit restless over the last few days. This morning she was pacing and her back legs seemed wobbly. She cries to me with an anxious voice. She was a bit constipated and threw up before passing it. Her appy is happy though.

The very didn't test her blood pressure. I'm going to go back today and ask them to do that. Is there anything else that I should ask for at the same time? She has some symptoms that could be hypertension. She had different sized pupils for a while when she had an URI recently. The vet out her on antibiotics. Her pupils returned to normal a while after the URI symptoms did.

I asked for tests initially because she was drinking and peeing more than normal although her bg has been within normal range.

Here are her test results that lead to diagnosis. I put in the international units too, just in case I made a mistake with conversion.

BUN 39.22 mg/dL (14.0 mmol/L)
Creatinine 1.94 mg/dL (148 umol/L)
Phosphorus 5.42 mg/dL (1.75 mmol/L)
Calcium 9.84 mg/dL, 4.92 mEq/L (2.46 mmol/L)
Potassium 4.9 mEq/L (4.9 mmol/L)
TCO2 22.8 mEq/L (22.8 mmol/L)
HCT 56% (0.56 L/L)

Specific gravity. 1.020 (no units on my results)
Urinalysis - no growth in 48 hours

Other outside of normal tests results:
Red Cell Count 11.47 ×10^6/uL (11.47 ×10^12/L)
Haemoglobin 17.6 g/dL
Basophils 0.07 ×10^9/L
Total Protein 9.04 g/dL
Albumin 3.52 g/dL
Globulin 5.52 g/dL
Glucose 169.37 mg/dL (9.4 mmol/L)
Total bilirubin 0.06 mg/dL (1 umol/L)
ALT 109 units/L
Cholesterol 208.88 mg/dL (5.41 mmol/L)

Ambg is due soon - I'll post an update. I've been up with her since 5 am.

Sorry that this is a little of topic today. I've joined the CRF support group from Tanya's site but I'm still under moderation prior to being able to post.

Do you have the reference ranges for the tests? The reference values may differ from lab to lab.

Hi Sienne, her labs are on her spreadsheet with reference ranges. I just did the conversions for you on her thread.

I also forgot to add in that she was grinding her teeth and shaking her head as she was pacing this morning. :-(

I don't know if you're already using this, but what about some Denamarin for the elevated ALT? I don't know if they have it in Australia. Anyone else have comments on this?

Most of the values suggest dehydration. They are not wildly out of range. The only value that I think shoul be monitored is the ALT. It's a big change since your last set of labs. If this were my kitty, I'd ask the vet about that result.

I've just had a very disappointing endocrinologist visit. She doesn't seem up to date with the research I've been reading in medical journals at all. She seems very diabetes focused and doesn't realise that there are other types of hypoglycaemia. It's time for an "educate-a-specialist" project. At least I've had practice with my "educate-a-vet" projects! :?

Sorry to get here so late.

If she's drinking/peeing more, you could possibly be looking at a hyper thyroid. You usually need a total T4 and free T4'to dx hyperthyroidism but sometimes the numbers will be normal but there are clinical signs of hyperT. For example, my Teddi showed symptoms of hyperT but her numbers were normal. Also, if there is anything else going on with the kitty, they can have euthyroid sick syndrome which means whatever else is going on, it masks the thyroid issue and numbers look normal. I don't know there is any thyroid issue going on with Angie but just passing along general info.

USG showing dilute urine can be an early indicator of CKD as long as she's not getting fluids for some reason because fluids will dilute the urine. Her creatinine, while in the normal range, still is indicative of minor azotemia along with the elevated BUN. And her phosphorus should not be any higher than 4.5.

One of the most important things to control in CKD is phosphorus. I would try to feed her foods that are lower in phosphorus. You also want to keep an eye to be sure she is not developing anemia. Right now her HCT looks great.

I hope she is feeling better and you can figure out these odd symptoms.

I didn't understand all of that but I'll read more so that I can. Angie seems normal again. I don't know what to do. Her blood pressure was 130 - he took two readings. He said to keep an eye on her and see how things go but he didn't see anything to be too concerned about at this point. He said that it could have been the physical pushing from constipation that made her throw up and then her strange behaviour afterwards may have been related to discomfort due to throwing up - maybe she had a bad taste in her mouth? He said that he could never be sure that there wasn't an underlying issue but that he didn't see another test that we should do at this point.

I spoke to mum this afternoon and we decided that I'm going to make an appointment with Dr Lisa Pierson to talk about an appropriate diet for her. It seems that preventative health is easier than treatment!

The vet said that ALT was only a little bit elevated and that it's something that has a fluctuates over a short time. He said that it on it's own elevated isn't such a concern because her other liver tests were good. It can be elevated for a number of reasons that can be temporary when the liver is working such as an infection or removal of toxins. He didn't want to do anything about it at the moment. If it continues to rise he would be concerned. He said that the evidence for the Australian version of Denarmarin in CKD cats was tenuous. He didn't want to consider it unless it continues to rise.

I don't like the attitude of waiting until things get worse before we do anything about it but he doesn't want to give unnecessary medication. I can understand that but I just know that she's not as well as she was a couple of months ago and I don't like not knowing.

I'm off to bed. I'll just have to see how things unfold. I'm glad to be getting a second opinion with Dr Pierson.
