3/14 Mickey update

He is fine today. No trouble breathing through the night.
The report is as follows:
Case details: Acute onset respiratory distress, open mouth breathing but pink, improvement on oxygen. PE: Obese, tachypneic and tachycardic. R and L lateral VD widened mediastinum, enlarged pulmonary vessels, mass to the heart?

Findings:
Thorax: Unlike the previous two examinations, there is indication of approx 3.0 cm diameter mass superimposed upon the cranial cardiac silouette or the RLR view, this opacity is not evident on the LLR and more important, it is not evident on the VD view. Because it is seen on the RLR view, one would expect the mass to be located in the left cranial lung lobe. The cardiac silhouette is normal size and shape on the opposite lateral views: the silhouette on the VD view is likely supplemented by a large volume of pericardial fat. The patient as on the others studies is excessively overweight. The caudal pulmonary vessels continue to be larger than normal as mentioned on the previous report. The caudal mediastinal widning is due to fat accumulation: no evidence of cranial mediastinal mass. THe tracheal lumen is uniformely norrowed in the cervical region on the RLR view. No plural abnormalities are seen.
Conclusion:
Thorax: Findings on RLR view are quite convincing of a large 3.0 mass superimposed upon the right atrial region of the cardiac silhoutette but the VD fails to confirm the presence of a right or left cranial lung lobe mass and there is no evidence of a mass associated with both the heart and pericardium. I question whether this mass is actually an artifact of the gross amount of thoracic body wall fat/tissue rather than a pulmonary mass. Beyond this finding. The inlarged caudal pulmonary vessels are again seen on the VD view but I suspect, since they were perviously mentioned, that HW disease has been ruled out. Overall, the thoracic radiographs are devoid of abmormalities responsible for clinical signs- there is no evidence of cardiac disease(although potential pulmonary hypertension cannot be ruled out with the large pulmonary vessels), no pulmonary abnormalities are seen now that a cranial mass is unlikely, there is no pleral effusion or pneumothorax and the trachea is normal diameter except for a caudal cervical narrowingon the RLR view. One could concider a lateral view of the upper airway to rule out problems in this region.

So essentually we do not know what happened. Perhaps some allergic reaction? But the vet feels if I did not get him in oxygen he would have expired while I was at work. There are no new foods, cleaning products out, I don't think he has been bitten by anything. An enigma?
I don't get it. But happy he is ok for now.

It may be something transient that can come and go.

The one repeated note I see is about excessive fat. You may already be working on helping Mickey lose weight, which all of us know can be pretty tough for people, much less our companions. If he can be helped to lose weight, it may help his breathing. You already may know that Dr Pierson has some notes about pet obesity on her web page Cat Info - Feline Obesity, but just in case, I put the link in.
 
Wow, a whole lot of words to tell you, "We don't know what happened". Frustrating, but at least his heart seems good and there were no other bad findings.

Have you had your windows open? Could something (fumes) have gotten to him.

Many years ago I had a 16 y.o. cat that seemed ok, sleeping in a chair, paid attention to me and my mom who came for dinner, but wasn't interested in eating. A while after my mom left the cat got up and jumped off the chair. She fell over, couldn't stand, cried out and dragged herself under the sofa, clearly frightened. I rushed her to the ER vet and by the time I got her there (20 mins) she was sitting up and alert and all they could say was, "she's a little dehydrated". I also could not find any cleaners or poisons she could have gotten into or anything else. This was over 20 years ago and I know more about health and wonder if she threw a clot, a tiny one, into her legs or had a mini-stroke. She ended up with bladder cancer dx about 8 months later and I had to PTS 2 months after that, so I wonder if she had cancer somewhere and a metastasis was floating around her blood stream and caused a stroke or some other blocked vessel. We just don't have the tests for them that we do for people.

I will continue to pray that it was a one time thing and he will be OK from now on. Give the big guy a pat from me. cat_pet_icon

Thanks Marcy! I saw your post on FB but I cannot answer anyone. We got a new policy at work - No social networking! And my computer at home is not hooked up - renovations, no electric yet! It is driving me crazy. lol

Sheila, I have had my windows open. But I just don't know. Right before he collapsed he was pooping. Like BJM said maybe something transient.

Thanks for the link BJM. His weight is a huge issue, no pun inteneded. He was 24lbs and decently sized in 2007 (first time he was DX) he went down to 18lbs and was stealth. After that he just kept going and going up. All of my cats eat the same food Low carb. He is the only one that is heavy. The others are anywhere from 5lbs to my 16 yr old cat that is 15lbs.
I need to get him moving more.
 
Peggy, it sounds like good news for the most part. Did they recommend a follow-up Ultrasound? That was just a radiograph reading, right? I am concerned because one view suggests a mass, the other view says it's just fat. There seems to be some confusion there, although they conclude it's just fat.

That would make me want to get a second opinion. And an ultrasound done by a licensed radiologist would probably be more conclusive.

I also have 1 cat who is much more overweight than her sister. The only reason I can think for her being so overweight is from the fat content of the Wellness food. Maybe some cats are more sensitive to fats, but I don't understand why her sister isn't as fat. I am taking her in at the end of the month to have a growth removed and a dental, so I will ask the vet, but her activity level has declined too. I think like people it gets hard to move with excess body weight. My only hope is that she will go outside and chase gophers again with the warm weather again. She's wanted outside every night when I came home this week.

Not sure if you can let Mickey outside or not when you're there. What kind of toys did he play with? Too bad they don't have a Biggest Loser show for pets, huh?

Mickey does not go outside. He hardly plays, just bats at things or hits the ball in the round a bout (the one that has the scratcher inside)
We only started feeding wellness core recently. It was always FF.
His sister Matilda doesn't eat wet food. Dry evo or taste of the wild. She weighs about 14.
That would be a dream to have a biggest looser program for pets! LOL
 
Although not as common as hyperthyroidism, it is possible for a cat to have hypothyroidism; might be worth testing, if you haven't already.

And have you ruled out other causes of large growth ... such as acromegaly???

Had to look those up before I answered your post.
We have had so many blood tests - a few of them were sent to Texas A&M
I have to call the vet to check but am pretty sure he has been tested for hypo and hyper.
Although with the hypothyroidism he does go outside of the box(poop) for the past year October. And does have dandruff which I related with the diabetes and being too large to groom himself. I do that for him.

Hyper: he has decreased interest in play, excessive sleeping, weight gain, seeking out warm places to sleep,
I did ask if the vet thought we needed to test for acro and he did not. But I can get him tested.
