This was a down day for Terra. But she's acting really strange now: she is oddly silent (usually she meows when she's hungry), she has been up and down the stairs several times, every few minutes, but is not doing anything productive when she comes downstairs, and acts somewhat agitated. She keeps running away from me, and stares as if she's hallucinating. Doesn't eat either, just tastes and then stares away from the food. I did not get a ketone test today.

I talked with my vet today and he wasn't inspiring confidence about testing for pancreatitis. He said that when cats get p-titis they don't go up and down as Terra has done, alternating a good day and a bad day, but ra†her, they get very critically sick. He said the best treatment for p-titis is to regulate insulin. None of these things sound like what I've read in this forum. I have put in a call to another vet and hope to talk with her tomorrow.
 
What we've often seen is that a cat will assume a "meatloaf" type position, be uncomfortable (if the episode is bad), not eat well, and is often nauseated and may vomit. Lethargy is also a symptom. Pancreatitis is a very painful condition.

Whether or not BG levels fluctuate due to pancreatitis isn't so much the issue. BG levels may be elevated due to pain.

Terra's is experiencing bounces. She drops into a good range (e.g., blues and greens) and then, since her body isn't used to spending time in these healthier ranges, her liver overreacts. As a result, the numbers spike up into higher ranges. It can take up to 72 hours for one of these bounces to clear.

Terra may be a little "off" because of these swings in numbers. She dropped from 422 to 146 in 6 hours. That's a huge drop. I'm not surprised she may not be feeling great.
 
My first thought was that she was feeling off because of the higher numbers today, and because of that quick drop yesterday.
 
there is "acute" pancreatitis and "chronic" pancreatitis. Cats with acute pancreatitis are usually very ill, as your vet said. However, studies have estimated that at least 30% of diabetic cats have chronic pancreatitis, and symptoms can range from very mild to very severe.

Feline Chronic Pancreatitis

Many of our cats here have at least occasional flares of chronic pancreatitis. Learning to recognize your own cat's early symptoms can go a long way toward being able to catch the flares before they become serious. If Terra is feeling "off," then it can't hurt to have the test done. If it comes back positive, then talk to your vet about keeping some fluids and possibly some pain meds on hand so you can treat Terra at the first sign of discomfort. Usually vets are more willing to work with you on those things once there is a diagnosis in hand. And, if it is not pancreatitis, then at least you will have ruled that out!
 
Good info for you today, Karen, about pancreatitis. I think it would be a good idea to find a vet who'll run the test for you. Remember you need either the fPLI or the specFPL.
