Whoomp there it is! He's back on the green floor. That was quite a cruise down from yellow, so I'm getting another test soon :mrgreen: :mrgreen: :mrgreen:

Edited to add this question: I understand that with Lantus, timing is important. If I absolutely can't do a shot on time, what are the options and how are they done?
Here is what I do understand already: Early shot=dose increase. Late shot=dose decrease.
It makes logical sense that the late shot is safer.
However, if I want to keep his BG down, can I divide the dose?
If I am gone from 4-9pm and our shot is normally at 7pm, could I do half of the dose early and half late? Can I do a drop early and the rest of the dose late?
I also understand I need to adjust the next few shots later to get back on track.

I understand that if anyone gives me advice it would not be considered ideal or standard of care. I'm just interested in understanding the pharmacology better so I can make an educated choice when circumstances aren't ideal.

Yikes Simon you could've meandered down you know, didn't anyone give you the memo about this being a marathon not a sprint!
 
The reason we don't encourage splitting the dose is due to the shed and overlap. It would be like having 2 curves where the peaks are identical except that one starts 2-hours later. Ultimately, this can cause problems. There are people in the Lev group that do this. I've not had experience with splitting the dose and can't give you input based on my experience.

If you were to shoot late, how big of a problem would it be over the next several days to get back on schedule?

I gave him literally one drop at 4pm because he was almost 200. I will probably give a s.25 at 9 and then adjust the next few days. I'm still interested in answers to add to my knowledge base, though!
 
What I've seen is that we usually opt to shoot late. It's safer. As long as you aren't completely locked in to your AM shot time over the subsequent days, the late shot is the best option (IMHO). If the AM schedule isn't flexible, we're usually suggested skipping.
 
i'd go with late or skipping also, depending on what it does for your schedule tomorrow. just seems risky to me to have the nadirs potentially overlapping, especially when you're working in lower numbers anyway.

i have opted to skip when i couldn't figure out another option. you can just call it a fur shot and let it go. seems safer to me.
