2/28 Max update

I haven’t posted anything in a while, but we’ve had some changes. I’ve been trying my absolute best to do all that I know to do for Max. It’s breaking my heart that despite all the things I have tried, he’s getting worse. Quickly at that. I noticed that he has become more picky about food for the past month, but I’ve been able to give him more variety until he finds something he will eat.

I took him to the vet yesterday because of the changes in him that I’ve been seeing. They did a blood pressure which was 170. He had labs which are on his SS. His weight is still pretty much stable, but a few ounces less than last time. Not concerned about that at this time though. His renal labs are a lot worse. All that I have fed is low phosphorus, by Weruva. He was eating Royal Canin renal diet up until the last 3 weeks when I read that RC, Purina and some other foods were making pets get sick and worse, so I stopped feeding that and he’s just on low phosphorus food now. I’m not sure what the right thing to do about his diet would be. Feed the RC renal or not???

His BP meds were changed back in January to Telmisartan and the amlodipine and enalapril were stopped. His blood pressure is still 170 at the vet’s, but they believe it is lower at home so they have not adjusted the med. He is getting sub Q fluids carefully 50 ml’s 2 or 3 times a week. He stopped drinking the Purina hydracare a while ago. The subq fluids seem to perk him up a little bit, as he’s otherwise so tired. He still will play for a few minutes at times and he’s still cuddly and purrs. Sometimes he likes to be by himself though and I respect that so I don’t bother him.

I am at a loss of knowing what to do at this point. My heart is breaking. It’s hard to just let him be a cat. I’m way too attached to Max. It hurts watching his health decline and I don’t know what else to do. Anyway this is where we’re at now. :(
 
:bighug::bighug::bighug::bighug: I’ve been in your shoes with Max and it is heartbreaking. You can feed low phosphorus food and I would feed any he likes. As I recall Weruva is pretty low calorie but my cats never liked it so I might be wrong. Will he eat baby food? You could supplement with it as it is low carb. That’s good news about the bp. My Max always was high despite amlodipine.

I wish I had a magic bullet!
 
I'm glad to see a Max update, to find out how he's doing.:bighug::bighug::bighug: You may find kidney values go up and down, depending on how hydrated he was that day. I do like that he's occasionally still playing. How old is Max now?

Stable weight is great! As for food, you can always add a phosphorus binder if you find a food he really loves. At this point, you feed them what they will eat. Of course the problem with binder is that it might add to his constipation. So you may need to up the Miralax. For liquids, I made Neko some chicken broth to tempt her to drink more. I cooked chicken to make it. Most commercial broths have veggies not good for cats(onions or garlic), though I've also seen some pet ones that might be good, just more expensive and if cooked with bones might be higher phosphorus. If he can't eat chicken, make broth with another meat.

I see his HCT is also creeping down. Are you giving him B vitamins to help? See Felinecrf website for dose and details.

Max is 14 years old now. I have been giving him B complex. Thanks for the suggestion on giving chicken broth. I have not tried that yet, but I will. I wasn’t sure about what else needed to be done about his HCT. His vet said she would contact NC State about his labs, but I’ve also called to see if we can get a sooner appointment than mid April. So just waiting to hear back.

Thank you. They just called and moved him up to March 5th. I boiled chicken and gave Max some of the broth. He loved it. Thanks for that suggestion. If you think of anything else, let me know. Still feeling overwhelmed, but I’m getting it together.

@tiffmaxee, thank you for your comment too. Max does not like baby food. We have tried chicken, turkey and ham back when he had a bad case of pancreatitis. Haven’t tried recently though.
 
Hi Lori, it’s nice to see you here. We’ve missed you and Max. I know how much you love your sweet boy. He’s so lucky to have you give him all the loving care you do.
Just want you to know we are thinking of both of you and sending love and big hugs of support and strength.
I hope you can get some help at your appointment at BC State.

@Max & Lori :bighug::bighug::bighug::bighug::bighug::bighug::bighug::bighug:I was getting worried not seeing you post. But you look at Max's SS and it looks so good, all that green and he's on a fraction of the insulin he was on when I first started watching! (10units).

I couldn't find it in your SS, but did you not have Max on a phosphorus binder at some point? I also noticed you have him now on Famatodine? Does he have tummy acid? Higher phosphorus can make a kitty feel like crap and cause tummy acid. I know I'm reaching for straws as I am no medical person. I'm sorry you are going through all of this, it is a very hard place to be. Please know we are all pulling for you and Max. We love you like family. I'm sending my strongest healing vibes and I pray the vet will have some suggestions.
 
Thanks Ruth. Sorry to have everyone worried, but I have just been trying to keep up with Max’s needs and to be honest I’ve been feeling beat up with worry, fatigue, and everything else. I’m sure you know the feeling. His insulin needs have come down a lot in a short amount of time. While I’m glad to see it, I can’t help but think something bad is behind it. Now I know he’s moved very quickly into stage 4 CKD. I had him on a phosphorus binder but the vet at NCSU said he didn’t need to be on that yet (back in January) so I stopped it. Now he does need it. He’s been vomiting some clear fluid in the early morning hours some day so I feel he does have acid. I’ve had a hard time and have wasted a lot of food trying to get him to eat. Seems like he only wants the bad stuff, the RC and the proplan. I’m too afraid to let him have that so I keep trying other things. It is exhausting mentally, emotionally, and physically but I am hanging in there for him.
 
@tiffmaxee Max needs an appetite stimulant and the Mirataz isn’t helping a lot. The vet is calling in Cyproheptadine for us. Have you used this as an appetite stimulant in cats? He is eating a little but it takes a lot of effort to find something he likes at this point. Thanks.

I have use d cyproheptadine and it’s my go to as an appetite stimulant. Start on a small dose and if it doesn’t work you can increase. I needed just a sliver for Max but Tiffany got 1mg. It can be given twice a day which I liked too. It’s the remedy should mirtazapine cause serotonin syndrome so can’t be given with mirataz. Do you think he could be nauseous? I dint recall if you have ever tried ondansetron for that. You can give both ondansetron and cyproheptadine together.

Yes he gets Ondansetron twice per day. Thanks for letting me know.
