Just a stab in the dark here with Sammy but have you ever thought that maybe the Lantus just isn't a good fit for Sammy?
I know so far Musette seems to be doing a 180 since I switched her over to Levemir. Just looking at Sammy's spreadsheet it looks like you are doing about the same thing I have been beating my head against a brick wall with, chasing up the dosing scale and then back down again only to go back up again when Musette was on Lantus.
Now since I switched insulin with her she is still higher than I would like her to be for the most part but she has smoothed out a lot and not bouncing all over the place, and she is still just getting settled in on the insulin switch, but it really has made a huge difference in her overall.
It could be just like with Musette Lantus just isn't the right insulin for Samwise. Just food for thought.
