Midas is currently in this 10th cycle after going up to 6.5 units. I was hoping that with a PMPS of 204 he would come down into blue and maybe green at the end of the cycle but so far he's 312 at +2. Advice please? (edit to add, he just came down a bit at +5 257)

He had blood in his stool 2 days ago and I'm still waiting to hear back from the vet's office.

I also need to come up with a game plan on how to proceed with tests for conditions that could be causing the need for higher doses of insulin.

This was being discussed in my thread here:
https://www.felinediabetes.com/FDMB/threads/2-9-midas-amps-158-blood-in-stool.259108/

So, I have questions.

I know he should have a urinalysis. But is that done before a urine culture? Do the results of the urinalysis determine if he needs a urine culture?

I know he should have blood work. I should see how his thyroid is at this point. I know that when this blood work is done, blood should be sent to Michigan State University for testing for Acromegaly and IAA. I am not clear on how testing for Cushing's Disease is done. Is this something my vet can do or does it need to be sent somewhere specific?

Am I missing anything else?

Thanks

Hi Colin,

There is some info in the stickies in the Acro/IAA/Cushing forum:
https://www.felinediabetes.com/FDMB/forums/acromegaly-iaa-cushings-cats.12/

I don't have an experience with these conditions so I will tag a few senior members who do:

@Wendy&Neko
@Sandy and Black Kitty
@JL and Chip

I think you are good for an increase to 6.5U tonight unless you get a blue preshot, in which case you may want to give him one more cycle on the dose. Wendy will confirm.

Has he had blood in his stool again? How is he otherwise? Eating, drinking, peeing, pooping okay?

I think a Urinalysis is done first and if there are signs of an infection, a C&S is required to confirm that.

Usually enough urine is drawn that you can do both urinalysis and a culture. Some vets will do the urinalysis, and see if there are bacteria present to decide if they then get the culture done. My vet liked to do both at the same time. A culture does take a few days to get results, so if you wait that's another day for the culture results. My vet also told me that she sometimes had cultures show something when the urinalysis did not. None of Neko's cultures ever found anything, at over $200 a pop. :blackeye:

Good luck with the 6.5 unit dose. With high greens on the previous dose, I would have held just 10 cycles.

I haven't been able to determine if he's had blood in his stool again. I have an appointment with my vet tomorrow so hopefully I'll learn more.

Other than the blood in his stool last Wednesday, he seems perfectly fine. Because the stool with the blood was pretty dry, I thought there may be a dehydration problem. However, I'm familiar with doing a tent test for dehydration and he's fine.

I do have a question about food I hope someone can help me with...
Over the past couple weeks, he is even more hungry than usual. He's been getting about 4.5 oz of food twice a day. He would definitely eat much more if I gave it to him. Can I give him more food? Would it negatively affect his BG numbers?

As always, I appreciate your help

how many calories per day is he eating?
Is he under weight? Overweight or just right?
Any history of ketones?

High BG indicates food is not being properly metabolized into usable energy so kitties with FD often are hungry in spite of eating. As BG gets better regulated the hunger subsides.

The other test is to check his gums. If they are tacky (sticky) rather than moist, it could mean he is dehydrated.

Like Sandy said, unregulated diabetic cats are always hungry because they cannot process their food. But his numbers have been better in the last 2 weeks than before. Does he get food only twice a day with his shots? Any smaller snacks in between?
