14/14 Bailey UK Advice on dosing prior to anaesthetic

Bailey has not been well this week,having daily visits to the vet,after extensive blood/urine testing he came to the conclusion that it was a UTI & a flare up of his pancreatitis.
During the night he vomited blood,so this morning off we go again to the emergency clinic,luckily the vet who had seen him all last week was on so he knew the case history (this is a very good experienced vet)He suspects that he might have an ulcer,and started him on Zantac,we decided that if vomited again he would put the camera down his throat and see what was going on,and to bring him in first thing tomorrow morning.If he is going to have an anaesthetic tomorrow he will not be able to eat after his PM shot,he is on 4 units of Lantus,I presume I need to reduce his dose,could you please advise me by how much? Poor bailey. I can't advise on the dosing at all but just wanted to say that my thoughts are with you.

In addition to the Zantac my cat remi also had antepsin for a couple of years to help protect against stomach ulcers. Just thought I might mention that in case it might be useful to you at some point. It's a tablet that dissolves when mixed with water and I used to just syringe it into his mouth twice a day.
 
Thank you for your kind reply,does Remi have ulcers?its been such a stressful week for Bailey,I am thinking that the diagnosis of a UTI & flare up of P'titis is a "red herring" and that the problem all along has been an ulcer.the strange think is he is hungry and eating well.
 
(((Diane))) - sorry to heal that Bailey might have to have anaesthesia. I'd ask the vet what they say about dosing in the morning. They may have an expectation. I've heard anything from 1/4 to 1/2 dose, and I've seen some people skip if kitty is low in the morning. Neko is going for dental next month, and the dental vet said 1/2 dose, but he's allowing me to feed her up to 6 AM, so she'll just get full breakfast a couple hours before AMPS. Last time she had a dental I did a 1/4 dose (no food after midnight) and she hit 4.2 on the Alphatrak just before going under. :o
 
When I had Spitzer (GA) in for a dental, my vet had him fast 8 hours and give half the insulin.

They can run low while under anesthetic, so use your judgement.

The problem being vet said no food after midnight tonight,he has his AM shot at 6am,so there is no way I can get intouch with vet before I take him in at 9am tomorrow,his numbers are not good as for the first few days of this week I reduced his insulin on advise from vet as Bailey had lost his appitite,and was eating very little.

we've been advised to give no food after midnight and a half dose at the regular time (7am) for morning procedures. If your vet gives you advice otherwise, though, it's a good idea to follow it. He may have expectations on the insulin and if that's what the vet knows what to work with, you'd want to follow his advice.

Be aware that cats can go low after the anesthesia, too, so some people skip the insulin that evening as well. I never had to with punkin - but perhaps with acro it's different.

I have just shot half his normal dose,it is 6am here in the UK and there is no way I can get to speak to vet as we are his first patient this morning,his numbers this last week have been high,the vet told me to reduce his dose as he was hardly eating for the first few days of last week,by midweek he was back to his normal amount of food so I started to ease his dose back up,but still not up to his normal dose,I suppose the reduction this morning is going to be another set back!

Morning. I hope all goes okay today. Remi was prescribed antepsin when he was vomiting a couple if times a day and was on a lot of tablets. The vet felt it would help protect against any ulcers from developing and treat any ones he may have. I continued this treatment until his diabetes diagnosis as he is always on a lot of tablets and they might irritate his tummy. I felt it did seem to calm things down somewhat, especially his throat which seemed quite irritated after the vomiting.

Well Bailey & I are both back from the vets,he decided not to scope him as he has not vomited since yesterday morning,he said he was sure he had a stomach ulcer bleed,and that he would like to try anti acids first,but if he vomited again he would definitely need to go the more invasive route of camera down the throat! Bailey is OK in himself,and his appetite has not been affected,so lets hope that the medication begins to sooth his tummy.

I'm glad Bailey is doing better. Here my vet would use carafate. It's a prescription drug for ulcers and a pain to give but stronger than the over the counter stuff. I wonder if they use it in the UK? Finers and paws crossed it won't be necessary but I would go that route before giving anesthesia. JMHO.

I don't recognise that drug,maybe it goes under another names in the UK.vet has prescribed human anti acid drug Omeprazole & Gaviscon,both in pill form,so that's going to be fun!!! have bought at the vets some German pâté that is in what I would describe as a toothpaste tube,apparently good for hiding the pill in so we shall see!
Bailey did the most massive mound of poo in the night,I would say 8/10 times his normal bowel movement,I put some in a sterile jar and took it into the vet,I was so afraid it was blood,vet said it wasn't so that is a bit of good news for a change.

Could be another complication from chronic pancreatitis - destruction of some of the enzyme producing cells in the pancreas, ie exocrine pancreatic insufficiency. May need to up the digestive enzymes you're giving.
 
glad he didn't have to get scoped, but i'm sure you'd like some answers. I hope the meds help him.

please let us know how he does in the next few days.
