Having a nasty bounce from good numbers yesterday. But all P's are in place and he seems happy and fine today. We are starting the Levemir tomorrow morning and I need some advice on dosing. My vet suggested to start at 2u BID. Is that too much? He is on 2.75u BID of Lantus now.

Hope everyone has a great day!

Re: 12/3 Napoleon AMPS 437 - need dose advice for Levemir

2.0 units is near enough to the recommended 30% reduction from 2.75u so that should be a good starting dose for you, on the Levemir.
This is more precautionary than anything, I have also heard that others have kept the dose the same and had no problems.

Be ready for an adjustment, as others have said, as any drop in BGs you may see after the changeover is likely to only be temporary.

HTH,
Perry.

Re: 12/3 Napoleon AMPS 437 - need dose advice for Levemir

I suspect 2.0u is fine. Let's see if Jill or Libby have any thoughts on the switch.

Re: 12/3 Napoleon AMPS 437 - need dose advice for Levemir

Thanks so much everyone  I think the 2.0u should be fine also. I am also curious to see what Jill or Libby suggests - would it be better to send them a PM?
I will be home with him all weekend to monitor and I have also ordered plenty of strips as my mom will be testing him next week while I am working.

Re: 12/3 Napoleon AMPS 437 +5~284 - need Lev dose advice

Nicole, 2 units should be fine. We only usually suggest a decrease because *sometimes* cats experience a big drop sometime during the first few cycles of Levemir (just like they do when they start Lantus or Lev for the first time). Some cats don't experience that at all. As long as you're able to monitor, I don't see any need to be any more conservative than that. Good luck with Lev!

Re: 12/3 Napoleon AMPS 437 +5~284 - need Lev dose advice

Thanks so much Libby! I will start with 2 units then. Quick question, when I opened the box, the vial of Levemir had a lot of bubbles at the top, is that normal?

Nicole...obviously I can't answer the bubble question but I wish you lots of luck with the conversion. Nice PMPS for Nap today!!!

Hi Nicole, While Rusty is not new to Levemir, I am, so I will be following Napoleon's condo closely to see what you are doing. I can't answer your question about the bubbles, either. Perhaps the vial got shook up in transit. I am using the flex pen and haven't had a problem with bubbles (anti jinx). Good luck tomorrow!

I just got my first vial of Lev a couple of weeks ago, and it looked like somebody had shaken it up. I was just hoping the people who say Lev is less fragile than Lantus were right. It took a few days, but the bubbles went away and so far her numbers seem fine.

Thanks Ella! I am hoping for some good numbers - consistently

Thanks Libby, that makes me feel much much better to know that it should be fine :-D
