Tonight we're going to complete the 6th cycle on 2.75 u but not happy with the numbers...seems like since the last couple increases his BG is slowly and constantly going up rather then down: could it be a case of insulin resistance, needs to reset, wrong insulin...?!
Just in case this may be important to know, Micio has a history of high ALT levels and numbers continuously went up and down: could that influence negatively on insulin? Or viceversa, could too much insulin make the ALT numbers rise?

Do I still need to increase by tomorrow morning?

Looks like he is seeing some glucose toxicity and he just needs to get to a good dose when he can breakthrough that temporary insulin resistance. You are on the right track with increasing by 0.25U every 3 days. Unless you see nadirs below 200 today, he is good for another increase tomorrow.

Insulin is a hormone and not a drug, so a cat's body can need a few months to learn the insulin and get regulated. Glargine is a good insulin for cats and we recommend considering a different insulin only after at least 6 months on it.

As far as reset is concerned - this is a pet theory of another FB group which invariably fails - we have seen several cats who have reset and are then battling glucose toxicity and stubborn high numbers. A reset will not work for Micio because he is not getting too much insulin. The only things that can cause a cat to be on too much insulin is (1) starting at too high a dose, (2) increasing by too large amounts so you bypass a good dose, (3) not testing enough to know how low the cat is going on that dose. None of these apply to him - you have started low, are increasing in multiples of 0.25U and getting enough tests to see how low he is going.

Hang in there! He will get to his good dose at some point. This journey really tests our patience! :bighug:

I don't see that much of a change in your kitty's numbers. It may help to keep in mind that there's a 20% variance in any test number so there's really no difference between a 275 and a 308. The difference in the color coding may make the difference seem larger.

I'm not seeing glucose toxicity. You've been steadily raising the dose. I do think it's a matter of you're not getting to an effective dose yet. Generally, glucose toxicity develops if you have held an ineffective dose for too long. You've been chipping away at Micio's numbers for only about 6 weeks. It can take a while to systematically get the dose to a point where the numbers are in the range you want them.

I absolutely agree about the "reset" issue. I'll take it one step further. It's a dumb idea (that the other FB group continues to promote). I've been here for a lot of years. I've never seen a reset be helpful. If anything, it puts a cat in high numbers for even longer.

Thank you for explaining, it makes sense. Although, I like to learn from anyone's opinions, experience and tips, there's a reason why I stick with FDMB :)
